HA330灌流器对高脂血症性急性胰腺炎患者炎性因子的影响

摘　　要：目的探讨HA330灌流器血液灌流治疗高脂血症性胰腺炎的临床疗效。方法回顾性分析长沙市中心医院2017年10月至2019年10月收治的60例高脂血症性急性胰腺炎患者,其中普通治疗组20例,HA230组20例,HA330组20例。记录患者入院时及治疗72 h后的血甘油三酯(TG),白介素-6 (IL-6),肿瘤坏死因子(TNF-α),C反应蛋白(CRP)水平,并进行评价与分析。结果普通治疗组、HA230组及HA330组间年龄、性别及治疗前血浆TG、IL-6、CRP水平均无明显差异(P>0.05)。普通治疗组与HA230组及HA330组治疗后TG水平有显著差异(P<0.05);HA230组与HA330组治疗后TG、CRP水平比较无明显差异(P>0.05);HA330组IL-6、TNF-α较HA230组低(P<0.05)。结论常规治疗和血液净化均能够有效降低TG水平,但HA230血液灌流及HA330血液灌流治疗效果更为明显,HA330血液灌流更能有效降低IL-6和TNF-α水平。【Objective】 To investigate the clinical effect of HA330 hemoperfusion apparatus in the treatment of hyperlipidemic pancreatitis(HLP). 【Methods】 Sixty patients with hyperlipidemic acute pancreatitis admitted in our department from October 2017 to October 2019 were analyzed retrospectively, including 20 cases in the routine treatment group, 20 cases in the HA230 hemoperfusion group and 20 cases in the HA330 hemoperfusion group. The levels of triglyceride(TG), interleukin-6(IL-6),tumor necrosis factor-α(TNF-α), C-reactive protein(CRP) were recorded at admission and 72 hours after treatment respectively.【Results】 There was no significant difference in age, gender and plasma levels of TG, IL-6 and CRP before treatment between the routine treatment group and the other two groups(P>0.05). There was significant difference in TG between the routine treatment group and the two hemoperfusion groups(HA230 and HA330 groups) after treatment(P<0.05). There was no significant difference in TG and CRP levels after treatment between HA230 group and HA330 group(P>0.05). The levels of IL-6 and TNF-α in HA330 group were significantly lower than those in HA230 group(P<0.05). 【Conclusion】 Both routine treatment and blood purification can effectively reduce TG level, but the treatment of HA230 and HA330 hemoperfusion were much more effective. Compared with other two groups, the levels of IL-6 and TNF-α were significantly decreased in HA330 group.
