摘 要:目的探究儿童支气管哮喘血清1,25二羟维生素D3[1,25-(OH)2D3]水平与支气管肺泡灌洗液中基质金属蛋白酶-9(MMP-9)、肿瘤坏死因子-α(TNF-α)、转化生长因子-β1 (TGF-β1)、白细胞介素-6 (IL-6)含量水平的关系。方法回顾性分析2015年12月至2016年11月于荥阳市妇幼保健院收治的儿童支气管哮喘患儿66例的临床资料,将其作为观察组,根据哮喘病情分成轻度、中度和重度,每组分别为20例、22例和24例,并收集同时期就诊30例非哮喘患儿资料作为对照组。比较儿童血清1,25-(OH)2D3水平及支气管肺泡灌洗液MMP-9、TNF-α、TGF-β1、IL-6含量变化及其相关性,以及支气管肺泡灌洗液细胞总数、巨噬细胞、炎症细胞数目变化。结果哮喘患儿血清1,25-(OH)2D3水平之间关系为轻度哮喘>中度哮喘>重度哮喘,而MMP-9、TNF-α、TGF-β1和IL-6水平则为重度哮喘>中度哮喘>轻度哮喘(P<0.05);支气管哮喘患儿支气管肺泡灌洗液MMP-9、TNF-α、TGF-β1、IL-6水平与血清中1,25-(OH)2D3之间的关系均为负相关(r=-0.539,-0.447,-0.461,-0.384;P<0.05);两组患儿支气管肺泡灌洗液细胞总数、炎症细胞数目进行比较,观察组患儿多于对照组,观察组巨噬细胞明显少于对照组(P<0.05)。结论支气管哮喘患儿血清1,25-(OH)2D3水平下降,支气管肺泡灌洗液中MMP-9、TNF-α、TGF-β1、IL-6水平上升,这种变化趋势对儿童支气管哮喘治疗具有重要的指导意义。【Objective】 To investigate the relationship between the level of serum 1,25-dihydroxyvitamin-D3[1,25-(OH)2D3]and levels of matrix metalloproteinase-9(MMP-9) and tumor necrosis factor-α(TNF-α), transforming growth factor-β1(TGF-β1)and interleukin-6(IL-6) in bronchoalveolar lavage fluid of children with bronchial asthma. 【Methods】 Clinical data of 66 children with bronchial asthma treated in Maternity & Child Care Center of Xingyang from December 2015 to November 2016 were retrospectively analyzed. They were included in the observation group and divided into mild(n=20), moderate(n=22) and severe asthma(n=24) according to the condition. Clinical data of 30 cases of children without asthma treated in the same period were collected as the control group. The changes of serum 1,25-(OH)2D3 levels and the contents of MMP-9, TNF-α, TGF-β1 and IL-6 in bronchoalveolar lavage fluid and their correlation were compared. The changes of total number of cells, macrophages and inflammatory cells in bronchoalveolar lavage fluid were also compared. 【Results】 Serum 1, 25-(OH)2D3 levels in children with asthma decreased significantly(P<0.05), and the levels decreased with the severity of the disease. However, levels of MMP-9, TNF-α,TGF-β1 and IL-6 increased significantly(P<0.05) and they increased with the severity of the disease. Serum 1,25-(OH)2D3 levels were negatively correlated with levels of MMP-9, TNF-α, TGF-β1 and IL-6(r=-0.539,-0.447,-0.461,-0.384;P<0.05). The total cell count and inflammatory cell count in bronchoalveolar lavage fluid of children with asthma were significantly larger, while macrophage count was significantly smaller than that in children without asthma(P<0.05). 【Conclusion】 Serum 1, 25-(OH)2D3 levels in children with bronchial asthma are negatively correlated with levels of MMP-9, TNF-α, TGF-β1 and IL-6 levels in bronchoalveolar lavage fluid, which is of guiding significance for treatment of bronchial asthma.
