吲哚美辛栓致严重神经系统不良反应并抽搐1例  

One case of severe neurological adverse reactions and convulsions caused by Indomethacin Suppository

摘　　要：1例33岁女性患者因痛经给予处方吲哚美辛栓2/3枚,用药半小时后出现头晕、喘促不适、手足麻木、抽搐、角弓反张、颈项强直、严重的电解质紊乱、呼吸性碱中毒等症状。动脉血气分析:pH 7.459,PCO 221.1 mmHg,PO 2144.2 mmHg,K+2.94 mmol·L^-1。快速地西泮10 mg肌内注射,症状逐渐缓解。4 h后再次发作,快速地西泮5 mg肌内注射,苯巴比妥100 mg静脉注射,症状逐渐缓解,恢复正常。考虑吲哚美辛栓不良反应,患者症状未再出现,第3天即出院。随访3个月,患者未见复发。A 33 year old female patient was prescribed 2/3 of Indomethacin Suppository once dose for dysmenorrhea.Patient had adverse reactions such as dizziness,wheezing discomfort,numbness of hands and feet,convulsions,opisthotonus,neck stiffness,severe electrolyte imbalance,respiratory alkalosis etc just half hour after insert the medication.Arterial blood gas analysis:pH 7.459,PCO 221.1 mmHg,PO 2144.2 mmHg,K+2.94 mmol·L^-1.The reaction relief after 10 mg diazepam IM once dose,yet reoccurred four hours later.After another 5 mg diazepam IM once dose and phenobarbital 100 mg IV dose,adverse reactions eased off and patient back to normal.Suggesting indomethacin induced adverse reactions.Patient discharged on the third day with no adverse reactions.And it didn′t happen after the following three months.
