达州市通川区首例新型冠状病毒肺炎本地病例流行病学调查

摘　　要：目的对达州市通川区首例新型冠状病毒肺炎(简称"新冠肺炎")本地病例开展流行病调查,为通川区新冠肺炎疫情防控提供参考。方法依照国家方案要求,采用现场流行病学方法对2020-02-02达州市通川区某医院报告的1例新冠肺炎确诊病例开展个案调查、采样检测和追踪管理密切接触者。结果2020-01-24患者出现身体不适,25日开始咳嗽和喘气,27日至31日在通川区某医院以感冒输液治疗5 d,因症状未缓解,遂住院治疗,31日22时隔离治疗,2月2被诊断为新冠肺炎确诊病例;自述无外出史和确诊病例接触史,发病前5 d与从浙江途径湖北和重庆返家大哥有密切接触,2月5日患者大哥呼吸道标本新冠病毒核酸阳性,诊断为无症状感染者。结论患者因接触无症状感染者而感染新冠病毒发病,无症状感染者有传染性;患者发病后多次就医,却未能及时隔离治疗,医院未严格落实"四早",需进一步加强能力建设,严防境外输入引发本地病例发生。Objective An epidemiological survey was conducted on the first local case of novel coronavirus pneumonia,to provide a reference for the prevention and control of novel coronavirus pneumonia in Tongchuan district.Methods According to the requirements of the national plan,field epidemiological methods were used to carry out case survey,sampling,testing and tracking management of close contacts of one confirmed case of coronavirus pneumonia reported by a hospital of Tongchuan district Dazhou city 02/02/2020.Results The patient developed physical discomfort on January 24 th,and started coughing and wheezing on the 25 th.He was treated with infusion for a cold in a hospital in Tongchuan district for 5 days from the 27 th to the 31 st.Because the symptoms were not relieved,he was hospitalized and isolated for treatment at 22 o’clock on the 31 st.He was diagnosed as a novel coronavirus pneumonia case on February 2 nd.There was no history of outgoing history and contact history of confirmed cases.Five days before the onset,he had close contact with his brother who returned from Zhejiang to Hubei and Chongqing.On February 5 th,the patient’s older brother’s respiratory tract specimen was positive for the novel coronavirus nucleic acid.He was diagnosed as asymptomatic infection.Conclusion The patient was infected with novel coronavirus due to contact with asymptomatic infection person,and the asymptomatic infectors were contagious;the patient went to hospital several times after the onset of the disease,but failed to be isolated and treated in time,The hospital did not strictly implement the"four early".So it is necessary to further strengthened capacity,and strictly prevent the local cases caused by overseas input.
