摘 要:目的:探讨Fisher&Paykel MR850湿化系统联合密闭式吸痰在呼吸机相关性肺炎中的应用。方法:选取重症监护病房2019年1月至2019年10月经气管插管建立人工气道的120例患者,随机分为对照组和观察组,对照组60例使用传统的吸痰方式即开放式吸痰,气道湿化方式选择人工鼻被动湿化。观察组60例使用密闭式吸痰方式,吸痰管不与外界接触,湿化方式选择Fisher&Paykel MR850湿化系统。对两组患者痰液黏稠度和VAP发生率进行比较。结果:两组患者建立人工气道后使用不同的吸痰和湿化方式,第5天和第7天观察组痰液更为稀薄,第7天痰液黏稠度比较有明显差别(P<0.05);观察组患者VAP 5d、7d和9d的发生率分别为0.00%、3.33%和8.33%,比较差异有统计学意义(P<0.05)。结论:Fisher&Paykel MR850湿化系统联合密闭式吸痰,大幅度的降低呼吸机相关性肺炎的发生率,对提高危重患者治疗效果具有显著意义。Objective:To explore the application of?Fisher&Paykel MR850 humidification system combined with closed sputum aspiration in suction-associated pneumonia.Methods:A total of 120 patients in intensive care unit(ICU)were randomly divided into control group and observation group.Sixty patients in the control group were treated with open suction,the artificial nose was used to humidify the airway.In the observation group,60 cases were treated with closed suction,the suction tube was not contacted with the outside world,and the humidification method was Fisher&Paykel MR850 humidification system.The viscosity of sputum and the incidence of VAP were compared between the two groups.Results:The Sputum was thinner on the 5th day and 7th day,and the sputum viscosity was significantly different on the 7th day(P<0.05)The incidence of VAP in the observation group was 0.00%,3.33%and 8.33%at 5,7 and 9 days respectively(P<0.05).Conclusion:Fisher&Paykel MR850 humidification system combined with closed suction significantly reduces the incidence of aspiration-associated pneumonia,and has a significant effect on the treatment of critical patients.
