Herbert螺钉治疗腕舟状骨骨折20例疗效观察

摘　　要：目的回顾性分析采用Herbert螺钉治疗腕舟状骨骨折的临床疗效。方法本研究选取2012年7月至2017年9月在该院治疗的20例腕舟状骨骨折,术前均有明确腕关节外伤史。治疗方法均采用掌侧入路,取腕部经桡侧腕屈肌腱、桡侧血管鞘间隙进入,保护周围组织,轻度背伸腕关节暴露腕舟状骨骨折部,骨折解剖复位后采用Herbert螺钉内固定治疗,参照Mayo评分标准行腕关节功能评价。结果20例患者均获完整临床随访,术后均规范指导行患侧腕关节康复锻炼,Mayo腕关节功能评价:优11例,良8例,可1例。结论腕舟状骨骨折应用Herbert螺钉治疗,手术操作简单,骨折固定牢靠,术后腕关节功能恢复良好。Objective Retrospective analysis the clinical effect of Herbert screw in treatment of wrist scaphoid fractures.Methods In this study,20 cases of wrist scaphoid fractures treated in this hospital from July 2012 to September 2017 were selected.All patients had clear history of wrist trauma before surgery.The palmar approach was used in all cases,taking through the gap of flexor carpi radialis tendon and radial vascular sheath to protect the surrounding tissues.After slight dorsal stretch of carpal joint,the scaphoid bone fracture was exposed clealy.Internal fixation with Herbert screw was adopted after anatomical reduction of the fracture.Results All patients were followed up integrally,and every patient got postoperative rehabilitation exercises of wrist joint in a standardized manner.According to the Mayo standard:the results were 11 cases excellent,8 cases of good,1 case of modest.Conclusion The use of Herbert screw for the treatment of scaphoid fractures of the wrist is simple in operation,and the fracture is firmly fixed,and the wrist joint function recovers well after the operation.
