Q开关Nd:YAG倍频532nm激光联合光子嫩肤治疗面部雀斑疗效分析

摘　　要：目的:探讨Q开关Nd:YAG倍频532nm激光联合光子嫩肤治疗雀斑的疗效。方法:选取186例面部雀斑患者,随机分为观察组、对照1组和对照2组,每组62例,对照1组给予Q开关Nd:YAG倍频532nm激光治疗,对照2组给予光子嫩肤技术治疗,观察组给予Q开关Nd:YAG倍频532nm激光联合光子嫩肤治疗,评定三组外观疗效,对比治疗前后的VISIA皮肤图像指标、皮肤性状和血清内皮素1(ET-1)和血管内皮生长因子(VEGF)水平变化,对三组患者进行为期6个月的随访观察,记录原皮损处雀斑复发病例。结果:观察组的临床总有效率显著高于两对照组(P<0.05),对照1组显著高于对照2组(P<0.05);治疗后,三组的VISIA皮肤图像指标中色素斑、皱纹、纹理、毛孔及紫质沉淀共5项评分和皮肤性状指标中皮肤色泽、皮损及均一性评分均显著降低(P<0.05),三组的血清ET-1水平显著降低(P<0.05),VEGF水平显著升高(P<0.05),以上指标对比,三组间治疗后比较,差异均具有统计学意义(P<0.05),观察组显著高于两对照组,对照1组显著低于对照2组(P<0.05);观察组、对照1组及对照2组的原皮损处雀斑复发率分别为1.61%、12.90%及16.13%,三组间对比差异有统计学意义(P<0.05),观察组的复发率显著低于两对照组(P<0.05)。结论:Q开关Nd:YAG倍频532 nm激光联合光子嫩肤治疗能明显消除雀斑,改善患者肤质,且能有效减轻术后炎性色素沉淀,提高临床疗效,降低雀斑复发率。Objective To explore the efficacy of Q-switched Nd:YAG frequency doubling 532nm laser combined with photorejuvenation on freckles.Methods A total of 186 patients with facial freckles were randomly divided into observation group,control group 1 and control group 2,with 62 cases in each group.Control group 1 was given Q-switched Nd:YAG frequency doubling 532nm laser,and control group 2 was given photorejuvenation,and observation group was given Q-switched Nd:YAG frequency doubling 532nm laser combined with photorejuvenation.The appearance efficacy was evaluated in the two groups.The VISIA skin image indicators,skin traits and levels of serum endothelin-1(ET-1)and vascular endothelial growth factor(VEGF)were compared before and after treatment.The patients in the three groups were given 6 months of follow-up observation,and the recurrence of freckles at the original skin lesions was recorded.Results The total clinical effective rate in observation group was significantly higher than that in control groups 1 and 2(P<0.05),and the rate in control group 1 was significantly higher than that in control group 2(P<0.05).After treatment,the scores of 5 items of pigment spots,wrinkles,texture,pores and purple deposits of VISIA skin image indicators and scores of skin traits indicators of skin color,skin lesions and homogeneity were significantly decreased among the three groups(P<0.05),and the serum ET-1 level in the three groups was significantly decreased(P<0.05)while the VEGF level was significantly increased(P<0.05),and there were statistically significant differences in the above indexes among the three groups(P<0.05),and the levels in observation group were significantly higher than those in control groups 1 and 2,and the levels in control group 1 were significantly lower than those in control group 2(P<0.05).The recurrence rates of freckles at the original skin lesions in observation group,control group 1 and control group 2 were 1.61%,12.90%and 16.13%respectively(P<0.05),and the recurrence rate in observation group w
