摘　　要：目的探讨链球菌感染对儿童过敏性紫癜(HSP)Th1/Th2失衡及预后的影响。方法选取链球菌感染引起的HSP患儿32例(感染组),非感染因素引起的HSP患儿35例(非感染组),同期本院健康体检儿童30例(对照组),采用酶联免疫吸附(ELISA)法检测各组外周血IL-2、INF-γ、IL-4、IL-10水平。同时比较两组患儿的并发症及复发情况。结果两组患儿外周血IL-2、INF-γ水平均低于对照组,IL-4、IL-10水平均高于对照组,差异有统计学意义(P<0.05)。感染组与非感染组相比,感染组患儿外周血IL-2水平更低,IL-4、IL-10水平更高,差异有统计学意义(P<0.05);INF-γ水平差异无统计学意义(P>0.05)。感染组并发症及复发率均高于非感染组,差异有统计学意义(P<0.05)。结论链球菌感染可能通过加重过敏性紫癜Th1/Th2失衡,影响预后。Objective To investigate the effect of streptococcal infection on Th1/Th2 imbalance and prognosis in children with Henoch-Schonlein purpura(HSP).Methods Thirty-two children with HSP caused by streptococcal infection(infected group),35 children with non-infected HSP(non-infected group),and 30 healthy children(control group)were enrolled.The expression levels of IL-2,INF-γ,IL-4 and IL-10 in peripheral blood of each group were detected by enzyme-linked immunosorbent assay(ELISA).The complications and recurrence rate were compared between the two groups after treatment.Results The expression levels of IL-2 and INF-γin the peripheral blood of the infected group and the non-infected group were lower than those of the control group,and the expression levels of IL-4 and IL-10 were higher than the control group,the difference was statistically significant(all P<0.05).Compared with the non-infected group,the expression level of IL-2 in the infected group was lower,and the expression levels of IL-4 and IL-10 were higher,the difference was statistically significant(all P<0.05).There was no significant difference in expression level of INF-γbetween the two groups(P>0.05).The incidence of complications and recurrence rate were significantly higher in the infected group.The difference was statistically significant(P<0.05).Conclusion Streptococcal infection may affect the prognosis by aggravating the imbalance of Th1/Th2 in children with Henoch Schonlein purpura.
