阴道三维能量多普勒超声血管参数在子宫瘢痕妊娠治疗中的临床意义研究

摘　　要：目的探讨阴道三维能量多普勒超声血管参数在子宫瘢痕妊娠治疗中的临床意义。方法选取2018年1月至2018年12月唐山市妇幼保健院诊治的60例Ⅱ型CSP患者作为研究对象。根据患者接受手术方式的不同将其分为两组,行超声监测下清宫术者为A组(n=30),子宫下段切开病灶清除术为B组(n=30),子宫下段切开方式包括经阴道、腹腔镜和开腹3种。分别对两组患者三维能量多普勒超声参数VI、FI、VFI及术中出血量进行相关性分析。结果在两组患者中VI、FI、VFI与出血量均呈正相关关系(r>0,P<0.01),但VI的相关性最显著。当A组患者VI<25、B组患者VI<50时,出血量缓慢增加,当A组患者VI>25、B组患者VI>50时,出血量成快速上升趋势。且当VI<10时,两种术式的出血量均<100mL。结论阴道三维能量多普勒超声血管参数中VI、VFI、FI与出血量呈正相关关系,上述指标特别是VI值可用于瘢痕妊娠风险评估,可为临床上子宫瘢痕妊娠的治疗提供参考依据。亦降低未来子宫破裂及胎盘植入的发生率。Objective To investigate the clinical significance of transvaginal three-dimensional energy Doppler ultrasound vascular parameters in the treatment of cesarean scar pregnancy(CSP).Methods Sixty patients with typeⅡCSP treated in Tangshan Maternal and Child Health Hospital from January to December 2018 were enrolled in the study.According to the different surgical procedures,the patients were divided into two groups.Group A received curettage under ultrasound monitoring and group B received incision and removal of lesions in lower segment of uterus,including transvaginal surgery,laparoscopic surgery and trans-abdominal surgery.Three-dimensional power Doppler ultrasound parameters VI,FI,VFI and intraoperative blood loss were compared between the two groups and the correlation between ultrasound parameters and intraoperative blood loss was analyzed.Results There was a positive correlation between VI,FI,VFI and blood loss(r>0,P<0.05),and most significant in VI.When VI<25 in group A and VI<50 in group B,the blood loss volume showed a slow rise tendency,and when VI>25 in group A and VI>50 in group B,the blood loss volume rose steeply.When VI<10,the blood loss volume was less than 100 ml in both groups.Conclusions There are positive correlations between the transvaginal three-dimensional energy Doppler ultrasound vascular parameters VI,FI,VFI and intraoperative blood loss.The above indicators particularly VI can be used for the risk assessment of CSP and provide a reference for its treatment,which can also reduce the incidence of uterine rupture and placental implantation.
