糖肾煎对2型糖尿病肾病大鼠的肾保护实验研究

摘　　要：目的:探讨糖肾煎对2型糖尿病肾病(DN)大鼠的肾保护作用。方法:采用单肾切除+高脂饲料+小剂量链脲佐菌素(STZ)腹腔注射,结合行为干预(饥饱失常、劳逸无度、惊吓)建立2型DN气阴两虚型证大鼠模型,共60只。按照随机数字表法随机分为A组(假手术)、B组(模型)、C组(糖肾煎)、D组(贝那普利),每组15只。检测各组大鼠干预处理后血糖[空腹血糖(FBG)、餐后2 h血糖(PBG)]、尿素氮(BUN)、血肌酐(Cr)、24 h尿白蛋白(24 h-UAlb)、胱抑素c(Cys-c)及血、尿β2微球蛋白(β2-MG),比较各组大鼠肾组织病理特征。结果:与A组比较,B组BUN、Cr升高并不显著(P>0.05),FBG、PBG、24 h-UAlb、Cys-c及血、尿β2-MG均明显上升(P<0.05);与B组比较,C、D组24 h-UAlb、Cys-c及血、尿β2-MG均有明显降低(P<0.05);C、D 2组上述指标差异无统计学意义(P>0.05);肾脏组织病变程度由正常~严重依次为A组、C组、D组、B组。结论:中药复方糖肾煎能有效降低2型糖尿病肾病大鼠的血糖、24 h-UAlb、Cys-c及血、尿β2-MG,发挥肾保护作用。Objective:To explore the renal protective effects of Tangshen Decoction on type 2 diabetic nephropathy(DN)rats.Methods:A total of 60 type 2 diabetic nephropathy rats with deficiency of both qi and yin were established by single nephrectomy,high fat diet and small dose of streptozotocin(STZ)intraperitoneal injection combined with behavioral intervention(hunger and satiety disorder,excessive work and rest,fright)and were randomly divided into a group A(sham operation),a group B(model),a group C(Tangshen Decoction)and a group D(benazepril),with 15 rats in each group.Fasting blood glucose(FBG),2 h postprandial blood glucose(PBG),urea nitrogen(BUN),serum creatinine(CR),24-hour urinary albumin(24 h UAlb),Cystatin C(Cys-C),blood and urineβ2-microglobulin(β2-mg)were detected after intervention.The pathological characteristics of renal tissue were compared between the 2 groups.Results:Compared with group A,BUN and Cr in group B did not increase significantly(P>0.05),FBG,PBG,24 h-UAlb,Cys-c,blood and urineβ2-MG increased significantly(P<0.05);compared with group B,24 h-UAlb,Cys-c,blood and urineβ2-MG in group C and D decreased significantly(P<0.05);the above indexes in group C and group D had no statistical significance(P>0.05);renal tissue lesions from normal to severe were A group,C group,D group,B group.Conclusion:Tangshen Decoction,the Chinese patent medicine can effectively reduce blood sugar,24 h-UAlb,Cys-c,blood and urine groupβ2-MG in type 2 diabetic nephropathy rats,and has protective effects on early DN kidney.
