滋阴降火汤治疗多囊卵巢综合征的临床研究

摘　　要：目的:探讨滋阴降火汤治疗多囊卵巢综合征(PCOS)的效果。方法:选取2015年5月至2018年4月贺州市人民医院收治的PCOS患者126例作为研究对象,按照抽签法随机分为对照组和观察组,每组63例。对照组给予口服达英-35和二甲双胍,观察组在对照组基础上给予滋阴降火汤。比较2组患者的雌激素与胰岛素水平、月经周期、Ferriman-Gallway(F-G)毛发评分及临床疗效。结果:治疗后,2组血清促卵泡生成素(FSH)、黄体生成素(LH)、雌二醇(E2)、孕酮(P)、睾酮(T)、催乳素(PTF)及糖负荷后1 h胰岛素浓度(INS 1 h)、INS 2 h、空腹血浆胰岛素(FINS)和胰岛素抵抗指数(Homa-IR)水平均明显降低,且观察组性激素、胰岛素水平和Homa-IR低于对照组(P<0.05);治疗后,2组患者的月经周期和F-G评分均低于治疗前,且观察组低于对照组(P<0.05);观察组的总有效率(84.12%)明显高于对照组(66.63%)(P<0.05)。结论:滋阴降火汤联合常规西药治疗POCS,可显著改善患者性激素水平和胰岛素抵抗,改善月经周期,值得临床推广应用。Objective:To explore the clinical effects of Ziyin Jianghuo Decotion on the treatment of polycystic ovary syndrome(PCOS).Methods:A total of 126 patients with PCOS admitted to the Gynecology Department of Hezhou People Hospital from May 2015 to April 2018 were divided into a control group and an observation group according to random lottery,with 63 cases in each group.The control group was treated with oral diane-35 and metformin,and the observation group was treated with Ziyin Jianghuo Decotion on the basis of the control group.The levels of estrogen and insulin,menstrual cycle,score of Ferriman-Gallway(F-G)and clinical efficacy were compared between the 2 groups.Results:After treatment,the levels of follicle-stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E2),progesterone(P),testosterone(T),prolactin(PTF),1-hour insulin concentration(INS 1 h),INS 2 h,Fasting plasma insulin(FINS),insulin resistance index(Homa-IR)significantly decreased in both groups,and the sex hormone,insulin level and Homa-IR in the observation group were lower than those in the control group(P<0.05).After treatment,the menstrual cycle and score of F-G in the 2 groups were lower than those before treatment(P<0.05).The total effective rate of the observation group was higher than that in the control group(84.12%vs 66.63%,P<0.05).Conclusion:The treatment of POCS with Ziyin Jianghuo Decoction combined with conventional Western medicine can significantly improve patients′sex hormone levels and insulin resistance,and improve menstrual cycle,which is worthy of clinical application.
