摘 要:目的 探讨心肺复苏(cardiopulmonary resuscitation,CPR)后患者血清白细胞介素-10(interleukin-10,IL-10)、胱抑素C(cystatin C,CysC)及B型利钠肽(b-type natriuretic peptide,BNP)水平及与预后的关系.方法 选取2016年9月—2019年9月在徐州市第一人民医院行心搏骤停CPR救治的患者87例,根据预后情况分为存活组和死亡组,比较两组血清IL-10、CysC、BNP水平.结果 87例经CPR恢复自主循环者中,最终存活出院患者27例,死亡60例;存活患者复苏后24 h和48 h时IL-10分别为(36.64±9.68)pg/mL和(51.12±24.03)pg/mL,CysC分别为(2.22±0.97)pg/mL和(1.18±0.63)pg/mL,BNP分别为(120.03±21.15)pg/mL和(87.36±22.25)pg/mL,明显低于死亡者(P<0.05);存活患者复苏后24 h和48 h时急性生理和慢性健康状况评分Ⅱ(acute physiolog-ical and chronic health status scoreⅡ,APACHEⅡ)评分分别为(23.81±8.44)分和(17.71±4.12)分,明显低于死亡者(P<0.05);复苏后24 h时IL-10、CysC及BNP与APACHEⅡ评分呈正相关(r=0.320、0.401和0.454,P<0.05).结论 CPR后患者血清IL-10、CysC、BNP水平可能与患者预后有一定关系,值得进一步研究.Objective To investigate the relations of the levels of interleukin-10(IL-10),cystatin C(CysC),and b-type natriuretic peptide(BNP)with the prognosis after cardiopulmonary resuscitation(CPR)in patients with heart arrest.Methods 87 sex,age,and underlying disease-matched patients with heart arrest underwent CPR.24h and 48h after CPR all patients underwent Acute Physiology and Chronic Health Evaluation H(APACHE H);and peripheral venous blood samples were collected to undergo examination of IL-10 by ELISA,CysC by imrhunoturbidimetiy,and BNP by electrochemiluminescence.Pearson correlation analysis was used to analyze the relations of the levels of IL-10,CysC and BNP with the prognosis after CPR.Results 27 of the 87 patients survived and 60 died.24h and 48h after resuscita-tion,the levels of IL-10 of the 27 surviving patients were(36.64±9.68)and(51.12±24.03)pg/mL respectively,both sig-nificantly lower than those of the dead cases[(47.15±1)and(62.24±1)pg/mL respectively,both P<0.05];the level of CysC were(2.22±0.97)and(1.18±0.63)pg/mL respectively,bolh significantly lower than those of the dead cases[(5.10±1)and(3.22±1)respectively,both P<0.05];and the level of BNP were(120.03±21.15)and(87.36±22.25)pg/mL respec-tively,both significantly lower than those of the dead patients[(384.46±2)and(345.03±3)pg/mL respectively,both P<0.05].The scores of APACHE H 24 and 48 hours after resuscitation of the surviving patients were(23.81±8.44)and(17.71±4.12),both significandy lower than those of the dead patients[(33.87±19.15)and(35.60±5.84)respectively,both P<0.05].Pearson correlation analysis showed that 24 hours after resuscitation,the IL-10,CysC and BNP levels were positively correlated with APACHE Ⅱ(r=0.320,0.401 and 0.454,P<0.05).Conclusion The serum levels of IL-10,CysC,and BNP may be related to the prognosis of patients after CPR.
