摘 要:目的 慢性伤口是多学科病因诱发的皮肤缺损,需要多学科合作共同来完成伤口的复杂治疗.构建"多学科合作的医护一体化伤口诊疗分级与分流模式".解决了疑难重症伤口患者的住院诸多难题.方法 优化诊疗流程,建立一站式会诊平台医护一体化、分级与分流工作机制.结果 从2017年—2019年,治愈的疑难慢性伤口患者增加125%,其中重症糖尿病足患者增加119%.2019年合作治疗及分级分流患者120人次.伤口治疗周期得到控制.培训了43家合作医院96名专科护理人员.结论 该模式经过两年的临床运转与实践,是可行的、有效的,值得推广.Objective Chronic wounds are skin defects induced by multidisciplinary etiology,requiring multidisciplinary cooperation to complete complex wound treatment.The establishment of a"multidisciplinary cooperative medical-care integrated wound diagnosis and treatment grading and triage model"has solved many problems for patients with difficult and severe wounds.Methods Optimize the diagnosis and treatment process,and establish a one-stop consultation platform that integrates the classification and triage of medical care.Results From 2017 to 2019,the number of patients with difficult chronic wounds healed increased by 125%,among which patients with severe diabetic foot increased by 119%.In 2019,there were 120 patients in cooperative treatment and grading triage.The"wound treatment cycle has been controlled.In addition,a total of 96 specialist nurses from 43 partner hospitals were trained.Conclusion After two years of clinical operation and practice,this model has proven to be feasible,effective,and worthy of promotion.
