摘　　要：目的探讨胎膜早破患者胎膜组织中人β防御素3(hβD3)与Toll样受体2(TLR2)蛋白表达的关系,并分析其临床意义。方法选取2017年2月至2019年3月宜宾市第二人民医院诊治的173例胎膜早破患者(研究组)和102例足月正常分娩产妇(对照组)的临床资料进行回顾分析。取两组胎膜破口处的组织采用免疫组化法检测hβD3、TLR2蛋白表达,对比结果;分析研究组患者胎膜组织中两种蛋白表达的相关性;对比研究组中不同破膜时间患者、早产患者和足月产患者、是否合并绒毛膜羊膜炎患者的胎膜组织hβD3、TLR2蛋白表达。结果研究组与对照组胎膜组织中hβD3、TLR2蛋白表达分布情况对比差异均具有统计学意义(均P<0.05),且研究组胎膜组织中hβD3、TLR2蛋白表达阳性率均明显高于对照组,差异均具有统计学意义(均P<0.05);研究组胎膜组织中hβD3、TLR2蛋白表达分布呈强烈正相关;破膜时间16h^24h和>24h患者胎膜组织hβD3、TLR2蛋白阳性率均高于破膜时间<16h患者,差异均具有统计学意义(均P<0.05),破膜时间>24h患者胎膜组织hβD3、TLR2蛋白阳性率均高于破膜时间16h^24h患者,差异均具有统计学意义(均P<0.05);早产患者胎膜组织hβD3、TLR2蛋白阳性率均高于足月产患者,差异均具有统计学意义(均P<0.05);合并绒毛膜羊膜炎患者胎膜组织hβD3、TLR2蛋白阳性率均高于未合并患者,差异均具有统计学意义(均P<0.05)。结论胎膜早破患者胎膜组织中hβD3、TLR2蛋白阳性表达率高,二者有强烈的正相关性,且与破膜时间、早产、合并绒毛膜羊膜炎均有密切关联。Objective To investigate the relationship between the expression of humanβ-defensin 3(hβD3)and Toll-like receptor 2(TLR2)in patients with premature rupture of membranes(PROM)and to analyze its clinical significance.Methods The clinical data of 173 patients with PROM and 102 cases of full-term normal delivery were retrospectively analyzed and recorded as study group and normal group accordingly.The expressions of hβD3 and TLR2 were detected by immunohistochemistry in the tissues of the rupture of fetal membranes of the two groups,and the results were compared.The relationship between the expression of the two proteins was analyzed.The expressions of hβD3 and TLR2 in the fetal membranes of patients with chorioamnionitis,premature delivery and full-term delivery were compared.Results The expressions and distribution of hβD3 and TLR2 in the fetal membranes of the study group and the normal group were significantly different(P<0.05),and the expressions of hβD3 and TLR2 in the fe-tal membranes of the study group were significantly higher than those of the normal group,all with statistically significant differences(P<0.05).The expressions of hβD3 and TLR2 in the fetal membranes of the study group were strongly positively correlated.The positive rates of hβD3 and TLR2 protein in fetal membranes of patients with rupture time of 16h-24h and>24h were higher than those of patients with rupture time of 16 h(P<0.05),and the positive rates of hβD3 and TLR2 protein in fetal membranes of patients with rupture time of>24h were higher than those of patients with rupture time of 16h-24h(P<0.05).The positive rates of hβD3 and TLR2 protein in the membranes of preterm women were higher than those of full-term women(P<0.05).The positive rates of hβD3 and TLR2 protein in fetal membranes of patients with chorioamnionitis were higher than those of patients without chorioamnionitis(P<0.05).Conclusions The positive expression rates of hβD3 and TLR2 protein in patients with PROM are high,which have a strong positive correlation wi
