摘  要:目的通过MSCT测量膝关节胫骨结节-股骨滑车沟距离(TT-TG)与胫骨结节-股骨髁间窝距离(TT-PD)并进行对照,评价2种方法在膝关节不稳中的诊断价值.方法选取男性及女性患者各40例,对双膝关节同一层面的TT-TG值与TT-PD值进行测量并采用SPSS分析;比较不同性别及左右膝关节的TT-TG值与TT-PD值.结果(1)对于股骨滑车沟各个层面TT-TG值之间的差异无统计学意义(P>0.05),相对应层面TT-PD值之间差异无统计学意义(P>0.05);(2)同一层面TT-TG值与TT-PD值之间的差异有统计学意义(P<0.05),且2组数据呈正相关,且TT-PD值偏大.(3)同性别双膝差异无统计学意义(P>0.05),同侧膝关节男女差异有统计学意义(P<0.05).(4)对TT-TG值、TT-PD值组内比较时发现左膝略小于右膝,男性大于女性.结论在股骨滑车沟的任意层面测量TT-TG值、TT-PD值均可作为评价参数,但TT-PD值略偏大.在对不同性别的双侧膝关节TT-TG值、TT-PD值对照时发现两者的左侧数值略小于右侧,男性两者的数值均比女性偏大.TT-TG值、TT-PD值能够反映髌骨的稳定性.Objective To compare the tibial tuberosity-trochlear groove distance(TT-TG)and tibial tuberosity-posterior femoral dome distance(TT-PD)by MSCT measurement,to evaluate their diagnostic value on the patellofemoral instability.Methods Forty cases of male and 40 cases of female patients were included.The value of TT-TG and TT-*PD at the same level of bilateral knees were measured in each patient and the obtained data were analyzed by SPSS.The TT-TG values and the TT-PD values of different gender and bilateral knees were compared.Results(1)The difference of the TT-TG values among each level of femoral groove were no significant(P>0.05)and the difference of the TT-PD values among the corresponding levels were also no significant(P>0.05).(2)The difference between the TT-TG value and the TT-PD value at the same level was statistically significant(P<0.05)and positive correlated,and the TT-PD value was larger than the TT-TG value.(3)The difference between bilateral knees for the same gender were no significance(P>0.05),but the difference of the same side knee between male and female was statistically significant(P<0.05).(4)The TT-TG values and the TT-PD values of the left knees were slightly smaller than those of the right,and were larger in male than those in female,when compared within the group.Conclusion Both the TT-TG values and the TT-PD values measured at any level of femoral groove can be used to evaluate the patellofemoral instability,but the TT-PD values arc slightly larger than the TT-TG values.In comparison of bilateral knees of different gender,the TT-TG values and the TT-PD values of the left knees are slightly smaller than those of the right,and slightly larger for male than for female.Both the TT-TG values and the TT-PD values can be used to predict the stability of the patellar.
