硬膜外麻醉复合瑞芬太尼用于腹腔镜妇科手术麻醉观察

摘　　要：目的:探究硬膜外麻醉联合瑞芬太尼在腹腔镜妇科手术临床麻醉中的应用效果。方法:2018年1月-2019年1月收治实施腹腔镜手术的妇科患者80例,随机分为两组,各40例。观察组采用硬膜外联合瑞芬太尼麻醉;对照组采用单纯硬膜外麻醉。比较两组临床效果。结果:观察组麻醉诱导时和结束时心率和血压水平均高于对照组,气腹10 min时血压和心率水平均明显低于对照组,差异有统计学意义(P<0.05)。观察组平均手术时间和麻黄素使用率均明显低于对照组,差异有统计学意义(P<0.05)。观察组手术麻醉效果优于对照组,差异有统计学意义(P<0.05)。结论:硬膜外麻醉复合瑞芬太尼在妇科腹腔镜手术中的应用效果显著。Objective:To investigate the effect of epidural anesthesia combined with remifentanil on clinical anesthesia in gynecological laparoscopic surgery.Methods:From January 2018 to January 2019,80 cases of gynecological patients who underwent laparoscopic surgery were enrolled,they were randomly divided into two groups with 40 cases in each group.The observation group was anesthetized with epidural anesthesia combined with remifentanil.The control group was anesthetized with simple epidural anesthesia.The clinical effects of the two groups was compared.Results:The heart rate and blood pressure level of the observation group were higher than those of the control group at the time of anesthesia induction and end of anesthesia,the blood pressure and heart rate were significantly lower than those of control group at 10 min of establishing an artificial pneumoperitoneum,the differences were statistically significant(P<0.05).The average operative time and ephedrine utilization rate in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).The effect of surgical anesthesia in the observation group was better than that in the control group,the difference was statistically significant(P<0.05).Conclusion:The effect of epidural anesthesia combined with remifentanil in gynecological laparoscopic surgery is remarkable.
