摘　　要：目的评价顺行联合逆行尿道造影判断男性后尿道损伤的准确性和安全性。方法回顾性分析北京积水潭医院泌尿外科2013年6月至2018年6月收治的后尿道狭窄或闭锁的318例患者的临床资料。将患者术前尿道造影片上尿道狭窄段或者闭锁段测量的长度与术中的测量值进行比较。同时统计该检查并发症的发生情况。结果尿道狭窄者在尿道造影片上测量的狭窄长度[(0.8±0.4)cm]短于术中测量的长度[(1.2±0.3)cm,P=0.002]。尿道闭锁者在尿道造影片上测量的闭锁长度[(2.3±1.1)cm]长于术中测量的长度[(2.0±0.9)cm,P=0.07]。该检查方法可能造成的并发症主要包括:感染,血尿,疼痛和过敏。结论顺行联合逆行尿道造影安全并且操作方便,但尿道造影片上测量的后尿道狭窄或闭锁的长度与术中测量值存在偏差,术者制定手术方案时应充分考虑。Objective To evaluate the accuracy and safety of anterograde combined with retrograde urethrography in the diagnosis of male posterior urethral injury.Methods Clinical data of 318 patients with posterior urethral stricture or atresia admitted during June 2013 and June 2018 in the Department of Urology,Beijing Jishuitan Hospital were retrospective analyzed.The length of urethral stricture or atresia measured on the urethrogram and during operation were compared.The incidence of complications of such examination were also recorded.Results In the urethral stricture group,the average length of urethral stricture measured on the urethrogram was(0.8±0.4)cm,shorter than that measured in the operation[(1.2±0.3)cm,P=0.002].In the urethral atresia group,the average length of urethral atresia measured on the urethrogram was(2.3±1.1)cm,longer than that measured in the operation[(2.0±0.9)cm].Complications associated with such examination included infection,hematuria,pain and allergies.Conclusions Anterograde combined with retrograde urethrography is safe and simple to operate,but the length of posterior urethral stricture or atresia measured on the urethrogram is different from that measured in the operation.Therefore,surgeons should give full consideration to the differences when designing operation plans.
