摘　　要：载脂蛋白B是动脉粥样硬化性心血管疾病发生的重要危险因素,它和传统危险因素结合可提高动脉粥样硬化性心血管疾病的风险预测能力。载脂蛋白B在脂质代谢中起到重要作用,有助于某些异常脂蛋白血症的特异性诊断。在动脉粥样硬化性心血管疾病的防治中已经取得一系列证据,目前血脂管理指南开始推荐载脂蛋白B作为动脉粥样硬化性心血管疾病风险评估指标,以及药物干预靶标。载脂蛋白B在心血管疾病剩留风险及糖尿病、代谢综合征、高甘油三酯血症、极低密度脂蛋白-胆固醇升高等患者中具有重要意义。但目前临床医生对载脂蛋白B与动脉粥样硬化性心血管疾病的关系尚缺乏科学认识,限制了其临床应用。本文对载脂蛋白B与脂蛋白代谢及其在动脉粥样硬化性心血管疾病防治中的证据、争议及前景进行系统阐述。Apolipoprotein B is an important risk factor for atherosclerotic cardiovascular diseases.Its combination with traditional risk factors can improve the prediction ability for atherosclerotic cardiovascular diseases.Apolipoprotein B plays an important role in lipoprotein metabolism and contributes to the specific diagnosis of some dyslipoproteinemia.A series of evidences have been obtained in the prevention and treatment of atherosclerotic cardiovascular diseases.In recent guidelines apolipoprotein B is recommended as a risk assessment index for atherosclerotic cardiovascular diseases and a drug interventional target.Apolipoprotein B has shown significance in patients with cardiovascular disease residual risk,diabetes,metabolic syndrome,hypertriglyceridemia and very-low-density lipoprotein-chdesterol increase.Nonetheless,clinicians are in lack of scientific knowledge about the relationship between apolipoprotein B and atherosclerotic cardiovascular diseases,which limits its clinical application.Here,we systematically reviewed and analyzed the evidence,controversy and prospect of apolipoprotein B and lipoprotein metabolism in the prevention and treatment of atherosclerotic cardiovascular diseases.
