摘 要:目的探讨子午流注理论对术中唤醒麻醉下脑功能区病变切除术患者心率变异性的影响。方法选择择期行脑功能区病变切除术且需术中唤醒麻醉技术的患者70例,随机分成上午手术组和下午手术组,各35例。在麻醉诱导前30 min袁电针组选取两侧内关、合谷和足三里,持续电针刺激至手术结束,并监测患者术前12 h和术后24 h动态心电图。结果与术前基础值相比,两组术后24 h TP、HF、LF和VLF均显著降低,差异有统计学意义(均P<0.05)。术后24 h,下午手术组TP和VLF显著高于上午手术组(均P<0.05)。结论下午手术并且针灸治疗可以有效稳定脑功能区术中唤醒麻醉患者机体自主神经功能状态,减轻应激损伤。Objective To investigate the effect of acupuncture on heart rate variability after resection of brain functional region lesions under awaking anesthesia based on theory of midnight-noon ebb-flow.Methods 70 patients with brain functional region lesions undergoing selective operation under awaking anesthesia were randomly divided into a morning group and an afternoon group,with 35 cases in each group.Thirty minutes before anesthesia induction,the Neiguan,Hegu,and Zusanli on both sides were continued to take electroacupuncture stimulation until the end of the operation in the two groups.The muli-channel dynamic eletrocardiographic(DCG)data 12 h before and 24 h after the operation were acquired and transferred for computerized analysis by the software(DMS Holter 5.0 program).Results Compared with the basic data,the TP,HF,LF,and VLF decreased significantly after resection of brain functional region lesions under awaking anesthesia in both groups(all P<0.05).24 h after the operation,the TP and VLF were significantly higher in the afternoon group than in the morning group(both P<0.05).Conclusions Electroacupuncture stimulation combined with resection of brain functional region lesions under awaking anesthesia at afternoon can effectively regulate the patients’autonomic nervous function and reduce stress injury.
