摘  要:目的研究应用3D打印导板技术辅助人工全膝关节置换术(TKA)的术后疗效及对患者步态和功能的影响。方法选取2018年8月至2019年8月在本院接受全膝关节置换的患者60例,用Excel表随机分为两组,各30例。试验组术中采用3D打印导板辅助TKA治疗,对照组采用传统TKA治疗。对比2种手术方式对患者一般手术指标、步态分析指标、膝关节功能评分造成的影响。结果试验组手术时间、切口引流量分别为(66.23±5.29)min、(267.39±27.59)ml,均低于对照组(89.49±4.56)min、(447.39±81.73)ml,差异均有统计学意义(均P<0.05);术后1 d、3 d,试验组血红蛋白指标分别为(116.68±5.93)g/L、(108.49±4.89)g/L,均高于对照组(105.33±9.02)g/L、(88.35±5.38)g/L,差异均有统计学意义(均P<0.05);试验组步长、步速、支撑相膝关节最小屈曲角度、支撑相膝关节最大屈曲角度、摆动相膝关节最大屈曲角度分别为(0.53±0.11)m、(0.89±0.21)m/s、(2.18±1.56)°、(31.98±8.93)°、(43.29±9.24)°,均高于对照组(0.43±0.04)m、(0.76±0.11)m/s、(1.57±0.45)°、(27.39±7.99)°、(39.29±5.39)°,差异均有统计学意义(均P<0.05);术前,两组患者膝关节功能比较,差异均无统计学意义(均P>0.05);术后3个月,两组患者HSS评分、KSS临床评分比较,差异均无统计学意义(均P>0.05);试验组KSS功能评分为(87.39±2.87)分,高于对照组(82.39±4.29)分,差异有统计学意义(P<0.05)。结论2种手术方式的术后疗效存在显著差异,应用3D打印导板辅助TKA的一般手术指标和步态指标均优于对照组,能更加有效提升患者膝关节功能评分,能显著改善患者预后。Objective To study the effect of 3D printing guide plate on gait and efficacy of patients treated with total knee arthroplasty(TKA).Methods 60 patients who took TKA at our hospital from August,2018 to August,2019 were randomly divided into a control group and an experimental group according to Excel,with 30 cases in each group.The experimental group were treated with TKA assisted by 3D printing guide plate,and the control group with traditional TKA.The common surgical indicators,gait analysis indicators,and knee function score were compared between these two groups.Results The operation time was shorter and the drainage volume was lower in the experimental group than in the control group[(66.23±5.29)min vs.(89.49±4.56)min and(267.39±27.59)ml vs.(447.39±81.73)ml;both P<0.05].1 and 3 days after the operation,the hemoglobin levels were higher in the experimental group than in the control group[(116.68±5.93)g/L vs.(105.33±9.02)g/L and(108.49±4.89)g/L vs.(88.35±5.38)g/L;both P<0.05].The step length,step speed,minimum and maximum buckling angles of the knee joint in support phase,and maximum buckling angle of the knee joint in swing phase were higher in the experimental group than in the control group[(0.53±0.11)m vs.(0.43±0.04)m,(0.89±0.21)m/s vs.(0.76±0.11)m/s,(2.18±1.56)°vs.(1.57±0.45)°,(31.98±8.93)°vs.(27.39±7.99)°,and(43.29±9.24)°vs.(39.29±5.39)°;all P<0.05].There was no statistical difference in the knee function between these two groups before the operation(P>0.05).3 months after the operation,there were no statistical differences in the HSS score and clinical score of KSS between the two groups(both P>0.05);and the function score of KSS was higher in the experimental group than in the control group[(87.39±2.87)vs.(82.39±4.29);P<0.05].Conclusion There is a significant difference in postoperative efficacy between the two methods.The basic surgical indicators of TKA assisted by 3D printing plate are better than the control group’method;it is more effective in improving knee joint abilit
