摘  要:目的:探讨颈动脉钙化与大脑皮质及浅层白质缺血的相关性。方法:分析148例患者头颈部血管CT造影及全脑CT灌注成像资料,收集患者检查同期临床资料。依据颈动脉钙化积分(calcium score)分组:CS>0组与CS=0组:根据脑灌注改变分为异常组、正常组。分析各临床资料在颈动脉CS>0组与CS=0组间有无统计学差异:分析各临床资料、颈动脉钙化与脑灌注数据相关性:分析颈动脉CS>0组与CS=0组之间对脑灌注阳性率的差异有无统计学意义。结果:年龄、吸烟史、高血压、糖尿病、高血脂、TC、LDL及血糖在颈动脉CS>0组与CS=0组间有统计学差异,P<0.05。颈动脉钙化与同侧脑组织灌注异常具有显著相关性,P<0.05。卡方检验结果表明颈动脉CS>0组与CS=0组间对各脑组织灌注阳性率的显示有统计学差异。结论:关注颈动脉钙化与脑组织供血障碍相关性,有利于脑缺血性病变的早期诊断和干预治疗。Purpose:To study the correlation between carotid artery wall calcification and ischemia of cerebral cortex and superficial white matter.Methods:Images of head and neck CT angiography and whole brain CT perfusion of 148 cases were analyzed.Some clinical data were collected at the same time.The cases were grouped according to the carotid artery calcium score(CS):CS>0 group and CS=0 group;and these cases were also grouped according to the cerebral perfusion:normal perfusion group and abnormal perfusion group.Differences of all clinical data between CS>0 group and CS=O group were analyzed.The correlation between clinical data,carotid artery wall calcium score and cerebral perfusion data were analyzed.The difference of cerebral perfusion between CS>0 group and CS=O group was analyzed.Results:Age,history of smoking,history of hypertension,history of diabetes,history of hyperlipidemia,TC,LDL and blood glucose showed statistically significant differences between the two groups,P<0.05.There were significant correlations between carotid artery calcification and ipsilateral cerebral perfusion,P<0.05.Chi-square test showed that there was statistically significant difference in the positive rate of cerebral perfusion between calcified group and non-calcified group.Conclusion:To pay attention to the correlation between carotid artery wall calcification and cerebral tissue supply disorder is beneficial to the early diagnosis and intervention of ischemic lesions.
