摘 要:目的:肋骨骨折的诊断是临床工作中影像医生经常面临的挑战。本研究对比CT多种后处理技术包括容积再现(VR)、多平面重建(MPR)和骨拉伸(BR)在肋骨骨折诊断中的应用价值。方法:回顾性分析本院2018年3月至2019年2月收治的肋骨骨折可疑患者,并通过手术或CT随访等方式证实为肋骨骨折。所有检查均予CT扫描,获取薄层(1mm)图像进行VR、MPR、和BR三种方法后处理,观察肋骨骨折情况,比较三种后处理技术在肋骨骨折诊断中的检出敏感度和诊断用时。结果:符合入组条件的肋骨骨折60例,男29例,女31例,年龄22~72岁,中位年龄42岁,经手术或CT随访确诊肋骨骨折共281处。VR检出228处,MPR检出251处,BR检出272处,三种方法检出敏感度组间差异明显(P<0.05)。三种诊断方法用时分别为(7.6±1.7)分钟、(8.2±2.2)分钟、(3.5±1.5)分钟,BR用时明显少于VR、MPR。结论:CT后处理骨拉伸技术对肋骨骨折检出敏感度优于传统后处理技术(VR.MPR),且可缩短诊断用时,具有较高的临床应用价值。Purpose:The diagnosis of rib fracture is a constant challenge for radiologists in clinical practice.This study is to investigate the applications of CT post-processing techniques including volume rendering(VR),multiplane reformation(MPR),and bone reading(BR)in diagnosis of rib fracture.Methods:Our hospital ethics committee approved this retrospective study.Patients who were suspicious with rib fractures at the first visit and finally confirmed the diagnosis by surgery or follow-up CT scan were recruited.CT scans with thin slices(1 mm)reconstruction and postprocessing including VR,MPR,and BR were performed for the diagnosis of rib fracture.The detection rate and the time duration for the diagnosis of rib fractures were observed and compared between 3 post-processing techniques.Results:There were 60 patients matched with the inclusion and exclusion criteria including 29 males and 31 females with age rang 22-72 years(median age,42 years).Totally,281 fractures were confirmed by surgery or follow-up CT imaging.The numbers of fractures detected by VR,MPR,and BR were 228,251,and 272,respectively.The differences in detection rate among three methods were with statistical significant(P<0.05).The time duration for the diagnosis with 3 methods used were(7.55±1.67),(8.17±2.25),and(3.47±1.46)mins,respectively.The time duration for diagnosing rib fracture by BR was significantly less than that by VR and MPR(P<0.05).Conclusion:BR technique is superior to traditional post-processing techniques(VR,MPR)in the detection rate of rib fractures with shorten the diagnostic time.Thus,CT post-processing technique of BR will improve the diagnosis of rib fractures.
