摘  要:目的探讨阻塞性睡眠呼吸暂停(OSA)患者血清神经肽S(NPS)水平与微觉醒之间的相关性。方法选取经多导睡眠检测确诊为OSA的62例患者,以呼吸紊乱指数(AHI)作为分组标准,分为轻度组(18例)、中度组(20例)及重度组(24例),同期挑选32名健康者作为对照组。采集受试者一般资料、血清NPS水平、Epworth嗜睡量表(ESS)评分及PSG数据。分析血清NPS水平与微觉醒的相关性,探讨基于NPS的OSA相关微觉醒发生机制。结果(1)随着OSA病情的加重,血清NPS水平、ESS评分、BMI均增高(P<0.05)。(2)OSA各组与对照组比较,OSA患者微觉醒指数(MAI)、非快速眼动睡眠期1期睡眠(N1)均升高(P<0.05),N3降低(P<0.05)。(3)OSA患者血清NPS水平与MAI(r=0.827)、ESS评分(r=0.692)、AHI(r=0.789)、N1(r=0.707)呈正相关性(P<0.05),与N2(r=-0.679)、N3(r=-0.483)呈负相关性(P<0.05)。结论OSA患者血清NPS水平及MAI显著升高。OSA患者血清NPS水平与微觉醒之间具有相关性,但OSA微觉醒机制较复杂,二者之间的关系需进一步论证。Objective To explore the relationship between serum neuropeptide S(NPS)level and arousal in patients with obstructive sleep apnea(OSA).Methods 62 patients with OSA confirmed by polysomnography were divided into the mild group(18 cases),the moderate group(20 cases)and the severe group(24 cases),and 32 healthy people were selected as the control group.Their general data,serum NPS level,Epworth Sleepiness Scale(ESS)score and PSG data were collected.The correlation between serum NPS level and arousal was analyzed,and the mechanism of OSA related arousal based on NPS was explored.Results(1)With the aggravation of OSA,the levels of serum NPS,ESS and BMI were all increased(P<0.05).(2)Compared with the control group,the arousal index(MAI)and non rapid eye movement sleep phase 1 sleep(N1)of OSA patients increased(P<0.05),and N3 decreased(P<0.05).(3)The level of serum NPS was positively correlated with Mai(r=0.827),ESS(r=0.692),AHI(r=0.789),N1(r=0.707)(P<0.05),and negatively correlated with N2(r=-0.679),N3(r=-0.483)(P<0.05).Conclusion The levels of serum NPS and MAI increase significantly in OSA patients.There is a correlation between the level of serum NPS and the arousal in OSA patients,but the mechanism of the arousal in OSA is complex.The relationship between the two needs further study and demonstration.
