宫内注射卡前列素氨丁三醇对高危妊娠剖宫产术后的影响

摘　　要：目的宫内注射卡前列素氨丁三醇对高危妊娠剖宫产术后患者子宫动脉血流动力学指标及出血量的影响。方法将84例高危妊娠剖宫产术后患者随机分为观察组和对照组,每组42例。对照组产后行缩宫素肌肉注射和静脉滴注,观察组在此基础上增加宫体注射卡前列素氨丁三醇,观察两组产妇子宫血流动力学指标和产后出血情况。结果观察组产妇搏动指数(PI)、阻力指数(RI)、收缩期峰值流速/舒张末期流速(S/D)明显小于对照组(P<0.05);观察组产妇产后2 h出血量及12 h出血量均低于对照组(P<0.05)。结论宫内注射卡前列素氨丁三醇能够显著改善高危妊娠剖宫产术后患者的子宫动脉血流动力学指标,降低PI、RI、S/D水平,促进子宫平滑肌收缩,作用快速、持久,在降低产后2 h出血量及12 h出血量方面具有重要的意义。Objective To explore the effect of intrauterine injection of caprost triol on uterine artery hemodynam⁃ic index and bleeding volume of patients with high-risk pregnancy after cesarean section.Methods 84 patients with high-risk pregnancy after cesarean section were randomly divided into the control group and the observation group,42 cases in each group.The control group received muscle injection and intravenous drip of oxytocin,the observation group increased intrauterine injection of caprost triol on the basis of the control group.The uterine artery hemodynamic index and bleeding volume of the two groups were observed.Results The maternal pulse index(PI),resistance index(RI)and systolic peak velocity/end-diastolic velocity(S/D)in the observation group were significantly lower than those in the control group(P<0.05).The amount of postpartum hemorrhage at 2 h and 12 h in the observation group was less than those in the control group(P<0.05).Conclusion Intrauterine injection of caprost triol can significantly improve uterine artery hemodynamic index and bleeding volume of patients with high-risk pregnancy after cesarean section,reduce the level of PI,RI and S/D,promote the contraction of uterine smooth muscle and has important significance in reducing postpartum bleeding volume.
