同期TURP联合无张力疝修补术治疗BPH并腹股沟疝的效果及其对膀胱功能的影响

摘　　要：目的探讨经尿道前列腺切除术(TUPR)和无张力疝修补术联合治疗前列腺增生(BPH)并腹股沟疝的效果。方法选取我院收治的125例BPH并腹股沟疝患者,按简单随机化法分为观察组(63例)和对照组(62例),对照组分期行TURP和无张力疝修补手术治疗,观察组行同期治疗。比较两组手术相关指标、膀胱功能、术后并发症和腹股沟疝复发情况。结果两组患者手术时间和前列腺切除质量比较无差异(P>0.05),但观察组单个手术出血量较对照组少(P<0.05);观察组术后6个月Qmax、膀胱顺应性较对照组高,IPSS、RUV及最大逼尿肌压较对照组低(P<0.05);观察组术后并发症发生率和疝复发率均较对照组低(P<0.05)。结论同期TUPR和无张力疝修补术联合治疗BPH并腹股沟疝可通过减少术中出血量和降低术后并发症,改善术后膀胱功能及避免腹股沟疝复发。Objective To explore the effect of transurethral resection of the prostate(TUPR)combined with ten⁃sion-free hernia repair in the treatment of benign prostatic hyperplasia(BPH)and inguinal hernia.Methods 125 pa⁃tients with BPH and inguinal hernia treated in our hospital were selected and randomly divided into the observation group(63 cases)and the control group(62 cases).TURP and tension-free hernia repair surgery were performed in the observation group.The operation-related indexes,bladder function,postoperative complications and recurrence of ingui⁃nal hernia were compared between the two groups.Results There was no significant difference in the operation time and the quality of prostatectomy between the two groups(P>0.05);but the intraoperative blood loss in the observation group was less than that in the control group(P<0.05).Qmax and bladder compliance at 6 months after surgery in the observation group were higher than those in the control group;the IPSS,RUV and maximum detrusor pressure in the ob⁃servation group were lower than those in the control group(P<0.05).The postoperative complication rate and hernia re⁃currence rate in the observation group were lower than those in the control group(P<0.05).Conclusion The TUPR combined with tension-free hernia repair for BPH with inguinal hernia can improve postoperative bladder function,avoid recurrence of inguinal hernia and reduce intraoperative bleeding and postoperative complications.
