腱鞘纤维瘤的肌骨超声误诊分析

摘　　要：目的探讨肌骨超声在腱鞘纤维瘤诊断中的应用价值与误诊分析及病理研究。方法回顾性分析经病理证实为腱鞘纤维瘤的12例肿物的肌骨超声表现资料,记录患者的姓名、性别、年龄和肿物的位置、内部回声、大小、形态、边界、有无钙化、有无液化、和周围组织的关系、血流分级、后方回声等情况及手术病理结果。结果本研究的12例肿物,肌骨超声确诊为腱鞘纤维瘤7例,误诊为腱鞘囊肿3例,误诊为Morton氏神经瘤1例,误诊为结节性筋膜炎1例。腱鞘纤维瘤可出现在关节腔内,也可见于肌腱旁,超声表现为实性回声和囊实性回声,关节内外均可见,可伴有液化,不伴有钙化,血流0~3级均可出现,典型病理表现为短梭形细胞和狭长裂缝状血管。结论肌骨超声在腱鞘纤维瘤诊断中有重要的应用价值。Objective To investigate the application value and misdiagnosis reason of musculoskeletal ultrasound in the diagnosis of tendon sheath fibroma and pathology research.Methods The sonogrphic findings of 12 cases of tendon sheath fibroma that were identified by pathological examination were retrospectively analyzed.The patient’s names,gender,age,tumor location,interal echo,tumor size,shape,boundary,calcification,liquefaction,and its relationship with surrounding tissue,blood flow classification,rear echo and surgical pathological examination results were obsered and analyzed.Results Among the 12 cases of tumors in this study,7 cases were definitely diagnosed as tendon sheath fibroma by musculoskeletal ultrasound,and 3 cases were misdiagnosed as ganglionic cyst,and 1 case was misdiagnosed as Morton’s neuroma,and 1 case was misdiagnosed as nodular fasciitis.The tendinous sheath fibroma could occur in the joint cavity and also could be seen near the tendon,and ultrasound examination showed solid echopattern or cystic echopattern both in or outside joints,with liquefaction,without calcification,with 0~3 grade blood flow.Moreover the typical pathplogical manifestations were short shuttle cells and narrow,cracky blood vessels.Conclusion The musculoskeletal ultrasound has an important application value in diagnosis of tendon sheath fibroma.
