摘　　要：目的探讨血浆可溶性生长刺激表达基因2蛋白(sST2)水平在门诊管理慢性心力衰竭患者的预后意义。方法回顾性分析2018年1月至2019年6月在新疆军区总医院门诊按顺序接诊的110例心力衰竭患者的临床资料,所有患者均随访1年。按照1年内是否发生心血管事件分为无心血管事件组(n=81)和心血管事件组(n=29)。比较2组患者sST2水平和临床检测指标;进行Cox回归分析,探讨影响预后的危险因素;通过血浆sST2水平受试者工作特征曲线(ROC)和曲线下面积分析,判断sST2水平对心力衰竭患者心血管事件(住院和心源性死亡)的预测意义。结果与无心血管事件组比较,心血管事件组患者sST2水平显著增高,差异具有统计学意义(P<0.05)。单变量和多变量Cox回归分析提示血浆sST2水平增高是心力衰竭患者1年内发生住院/再住院和心源性死亡的危险因素。结论门诊检查心力衰竭患者血浆sST2水平高低能够有效预测评估心力衰竭患者的心血管事件危险情况。Objective To investigate prognostic significance of plasma soluble sST2(soluble growth stimulation expressed gene)level in outpatients with heart failure.Methods A retrospective analysis was made of the clinical medical data of 110 patients with heart failure who sought medical care in the outpatient of the hospital in order of sequence from January 2018 to June 2019.All the patients had one⁃year medical follow⁃up.The patients were divided into the non⁃cardiovascular event group(n=81)and the cardiovas⁃cular event group(n=29),depending on whether or not cardiovascular events occurred within 1 year.sST2 levels and clinical data in the 2 groups were analyzed retrospectively,and Cox regression analysis was made as well to explore related risk factors affecting progno⁃sis of the patients.Through analyses of the receiver operator characteristic curve(ROC)and area under the curve(AUC),the signifi⁃cance of plasma sST2 levels in the prediction of cardiovascular events in patients with heart failure was evaluated.Results As compared with that of the non⁃cardiovascular event group,the plasma sST2 level in the cardiovascular event group significantly elevated,with sta⁃tistical significance(P<0.05).Univariate and multivariate Cox regression analyses indicated that elevated serum sST2 level was a risk factor of hospitalization and re⁃hospitalization and cardiogenic deaths in patients with heart failure within 1 year.Conclusion The de⁃tection of plasma sST2 level could effectively predict cardiovascular events in outpatients with heart failure.
