扶正化瘀法对子宫腺肌病患者痛经症状及血清CA125的影响

摘　　要：目的探讨扶正化瘀法对子宫腺肌病患者痛经症状及血清CA125水平的影响,评价扶正化瘀法治疗子宫腺肌病继发性痛经的临床疗效与安全性。方法将2016年1月—2018年12月就诊的68例子宫腺肌病患者随机分为对照组(A)33例,观察组(B)35例,对照组患者予散结镇痛胶囊治疗,观察组予扶正化瘀方治疗。观察2组患者治疗前后痛经症状、中医证候评分、子宫体积及血清CA125水平。结果与本组治疗前比较,A、B 2组VAS评分及中医证候积分均降低,子宫体积均变小,CA125水平下降(P<0.01);与A组治疗后比较,B组VAS评分及中医证候积分均低于A组(P<0.01),B组子宫体积小于A组(P<0.05),B组CA125水平低于A组(P<0.01);B组总有效率(88.6%)优于A组(57.6%)(P<0.01)。结论扶正化瘀法能够改善子宫腺肌症患者临床症状,使子宫体积变小,降低CA125水平,疗效优于中成药散结镇痛胶囊。Objective To investigate the effect of fuzheng and removing blood stasis on dysmenorrhea and serum CA125 level in adenomyosis patients,and to evaluate the clinical efficacy and safety of fuzheng and removing blood stasis in treating dysmenorrhea secondary to adenomyosis.Methods 68 patients with adenomyosis who visited the department of gynaecology in our hospital from January 2016 to December 2018 were randomly divided into control group(A)of 33 cases and observation group(B)of 35 cases.The patients in group A took 4 sanjie analgesia capsules orally three times a day,while those in the observation group were given strengthening vital qi for removing blood stasis method twice a day for three menstrual cycles.Dysmenorrhea symptoms,TCM syndrome score,uterine size and serum CA125 level were observed in the two groups before and after treatment.Results Compared with the groups before treatment,VAS scores and TCM syndrome scores in group A and B both decreased,as well as uterine volume and serum CA125 level(P<0.01).Compared with group A,VAS score and TCM syndrome score after treatment in group B were lower than those in group A(P<0.01),uterine volume in group B was smaller than that in group A(P<0.05)and CA125 level in group B was lower than that in group A(P<0.01).The total effective rate of group B(88.6%)was better than that of group A(57.6%)(P<0.01).Conclusion This method can improve the clinical symptoms of patients with Aadenomyosis and reduce the uterine volume and the serum level of CA125.
