五皮饮方治疗体外受精-胚胎移植术后的卵巢过度刺激综合征的效果

摘　　要：目的比较五皮饮方与西药常规治疗卵巢过度刺激综合征的临床效果,探讨五皮饮方加减的作用机制。方法选择我院2018年1月~2019年1月收治的符合诊断标准的体外受精-胚胎移植术后的中重度OHSS患者40例,随机分为研究组与对照组,各20例。对照组采用常规人血白蛋白、血浆和(或)羟乙基淀粉注射液、和(或)右旋糖酐40葡萄糖注射液等扩容治疗、低分子肝素预防血栓以及孕激素黄体支持,研究组在对照组基础上加用五皮方饮中药治疗;比较两组住院时间、住院花费、痊愈率、缓解率、腹腔穿刺次数、治疗前后外周血雌二醇(E2)、血管内皮生长因子(VEGF)、红细胞压积(HCT)以及白介素(IL-6)水平。结果研究组痊愈率(40.00%vs 15.00%)、住院费用[(11535.95±2269.48)元vs(21355.55±7605.58)元]、住院时间[(8.20±2.98)d vs(13.20±4.55)d]及腹腔穿刺次数(1次vs 3次)均优于对照组,差异有统计学意义(P<0.05);治疗后,研究组血清IL-6、HCT水平低于而对照组,差异有统计学意义(P<0.05);研究组VEGF、E2水平低于对照组,但差异无统计学意义(P>0.05)。结论在五皮饮方的协同作用下,体外受精-胚胎移植术后的中重度OHSS患者缩短住院时间,减少住院费用,减少了放腹水的次数。其机理可能是通过奏理气健脾利湿消肿调理患者内环境,使气血通畅,脏腑阴阳调和,机体水液运化平衡,湿消肿、理气健脾,对血清内VEGF、IL-6细胞因子的表达进行调控,提高了OHSS的临床治愈率。Objective To compare the clinical effects of Wupiyin recipe and western medicine in conventional treatment of ovarian hyperstimulation syndrome,and explore the mechanism of Wupiyin recipe.Methods A total of 40 patients with moderate to severe OHSS after in vitro fertilization-embryo transfer who were admitted to our hospital from January 2018 to January 2019 were selected and randomly divided into a study group and a control group,with 20 cases each.The control group received conventional human albumin,plasma and(or)hydroxyethyl starch injection,and(or)dextran 40 glucose injection and other volume expansion therapy,low molecular weight heparin to prevent thrombosis,and progesterone luteal support.The study group was in the control group on the basis of treatment with Wupiyin recipe Chinese medicine;compare the two groups'hospitalization time,hospitalization expenditure,recovery rate,remission rate,number of abdominal punctures,peripheral blood estradiol(E2),vascular endothelial growth factor(VEGF),red blood cells before and after treatment packed product(HCT)and interleukin(IL-6)levels.Results The recovery rate of the study group(40.00%vs 15.00%),hospitalization expenses[(11535.95±2269.48)yuan vs(21355.55±7605.58)yuan],hospitalization time[(8.20±2.98)d vs(13.20±4.55)d]and abdominal cavity number of punctures(1 time vs 3 times)was better than that of the control group,the difference was statistically significant(P<0.05);after treatment,the serum IL-6 and HCT levels of the study group were lower than those of the control group,the difference was statistically significant(P<0.05);The levels of VEGF and E2 in the study group were lower than those in the control group,but the difference was not statistically significant(P>0.05).Conclusion With the synergistic effect of Wupiyin recipe,moderate to severe OHSS patients after in vitro fertilization-embryo transfer shorten the length of hospital stay,reduce the cost of hospitalization,and reduce the frequency of ascites discharge.The mechanism might be to regulate the
