摘 要:目的构建老年慢病住院患者营养不良风险评估系统,为制定临床护理决策提供依据。方法依据奥马哈问题分类系统,通过文献回顾、患者访谈,制定评估系统初稿,采用Delphi专家咨询法修订评估内容,并运用层次分析法确立指标权重。结果 2轮专家咨询积极系数为90%和100%,专家权威系数为0.81和0.85,各级指标专家意见协调系数分别为0.401、0.382、0.357(均P<0.05)。确立的评估系统包含4个一级指标、15个二级指标和45个三级指标。结论本评估系统具有一定的科学性与可靠性,可用于评估患者生理、健康行为、心理社会、环境领域的营养风险问题。Objective To construct malnutrition risk assessment system for hospitalized elderly patients with chronic disease,and to provide basis for making clinical nursing decisions.Methods The initial version of assessment system was developed through literature review and patient interview on the basis of Omaha classification system.Delphi expert consulting was used to revise the content.The analytic hierarchy process was used to establish the weight of indicators.Results The positive coefficients for 2 rounds expert consultation was 90%and 100%,the authority coefficient was 0.81 and 0.85,and the coordination coefficient of expert opinions was 0.401,0.382 and 0.357(P<0.05).The assessment system included 4 first-level indicators,15 second-level indicators,45 third-level indicators.Conclusion This assessment system is scientific and reliable,and can evaluate patients'physiological,health behavior,psycho-social and environmental risks of malnutrition.
