孕晚期会阴热敷和按摩降低严重会阴撕裂伤的研究

摘　　要：目的通过对孕晚期孕妇会阴热敷和按摩,对孕妇的会阴撕裂伤的降低影响效果进行评价,从而研究热敷和按摩的应用效果。方法方便选取该院在2016年7月-2019年7月间收治的处于孕晚期的通过阴道分娩的120例初产妇作为研究对象,其中孕36周开始进行会阴热敷和按摩60例设为观察组,其余60例产妇设为对照组。观察组产妇在胎头拨露、感受到会阴发生伸展时,使用温热的生理盐水浸泡纱布,使用浸泡后的纱布热敷会阴部,按摩会阴处,比较两组产妇的会阴疼痛程度、会阴损伤情况以及会阴侧切率进行、产后会阴的肿胀程度进行比较以及护理总满意度。结果观察组会阴疼痛程度与对照组进行比较,观察组疼痛程度更低;观察组的会阴裂伤产妇有51例,会阴侧切概率为21.67%(13/60),对照组会阴裂伤产妇有57例,会阴侧切概率为26.67%(16/60),组间比较差异有统计学意义(χ^2=4.928,P<0.05);观察组会阴肿胀程度低于对照组;观察组产妇的愈合时间为2.6 d,对照组产妇的愈合时间为4.9 d;观察组的护理总满意度为100.0%(60/60),对照组护理总满意度为83.3%(50/60),组间比较差异有统计学意义(χ^2=10.909,P<0.05)。结论孕晚期的产妇提前到36周开始通过热敷以及按摩会阴部,可以有效地降低会阴发生严重的撕裂伤,产妇的疼痛感更低,得到显著的护理总满意度。Objective To evaluate the effect of reducing the impact of perineal lacerations on pregnant women during the third trimester of perineal hot compress and massage,so as to study the application effects of hot compresses and massages.Methods From July 2016 to July 2019 in the hospital,120 primiparas in vaginal delivery in the third trimester were convenienty selected as research maternal.Among them,60 cases of perineal hot compress and massage started at 36 weeks of pregnancy as the observation group,and the rest 60 maternals were set as the control group.When the fetal head was exposed and the perineum was stretched,the observation group maternal use warm saline to soak the gauze,use the soaked gauze to compress the perineum,massage the perineum,compare the pain level and perineal injury of the two groups of maternal.The situation and the rate of perineal lateral incision,the degree of swelling of the perineum after childbirth were compared,and the total satisfaction of nursing.Results Compared with the control group,the degree of perineal pain in the observation group was lower than that in the control group;there were 51 cases of perineal lacerations in the observation group,and the probability of lateral perineal incision was 21.67%(13/60),and 57 cases of perineal lacerations in the control group,The probability of perineal lateral incision was 26.67%(16/60),the difference between the groups was statistically significant(χ^2=4.928,P<0.05);the degree of perineal swelling in the observation group was lower than that in the control group;the healing time of maternal in the observation group was 2.6 d The healing time of maternal in the control group was 4.9 d;the total satisfaction of nursing in the observation group was 100.0%(60/60),and the total satisfaction of nursing in the control group was 83.3%(50/60).The difference between the groups was statistically significant(χ^2=10.909,P<0.05).Conclusion The maternal in the third trimester started to apply hot compresses and massage the perineum up to 36 weeks in a
