粉尘螨变应原滴剂联合低温等离子技术对成人OSAHS合并变应性鼻炎特异性IgE及IgG4的影响

摘　　要：目的探究粉尘螨变应原滴剂联合低温等离子技术治疗阻塞性睡眠呼吸暂停低通气综合征(SAHS)合并变应性鼻炎(AR)患者血清特异性IgE及IgG4水平的影响。方法选取我院于2015年1月至2016年7月收治的180例OSAHS合并AR患者,依据随机数字表法分为观察组和对照组,各90例。对照组给予低温等离子技术治疗,观察组在对照组基础上给予粉尘螨变应原滴剂治疗。比较2组患者治疗开始前与治疗满52周时评估症状评分与体征评分,血清白细胞介素(IL)-2、IL-4、IL-5、干扰素-γ(INF-γ)水平,及血清尘螨特异性IgE及IgG4。结果观察组脱落5例,对照组脱落0例,观察组治疗后的症状评分、体征评分显著低于对照组(t=16.307,P<0.01;t=57.284,P<0.01);治疗后观察组IL-2、IFN-γ显著高于对照组(t=7.579,P<0.01;t=17.618,P<0.01),观察组IL-4、IL-5显著低于对照组(t=25.472,P<0.01;t=16.873,P<0.01);观察组治疗后IgE低于对照组(t=9.708,P<0.01),IgG4高于对照组(t=35.690,P<0.01)。结论粉尘螨变应原滴剂联合低温等离子技术治疗OSAHS合并AR,能够促进患者免疫功能由Th2型免疫反应向Th1型转化,降低Th2型代表性细胞因子IL-4、IL-5和抗体IgE水平,升高Th1型代表性细胞因子IL-2、IFN-γ和抗体IgG4水平,纠正患者Th1/Th2免疫失衡,改善患者免疫状态,提高治疗效果。Objective To observe the influence of dust mite allergen combined with low-temperature plasma technology on the specific IgE and IgG4 of adult patients with OSAHS and allergic rhinitis.Methods From January 2015 to July 2016,a total of 180 cases of patients with OSAHS and allergic rhinitis were selected as the research subjects in our hospital,and were randomly divided into the observation group and the control group,90 cases in each group.The control group was treated with low-temperature plasma technology,and the observation group was given dust mite allergen on the basis of the control group.The symptom scores,signs scores,serum interleukin(IL)-2,IL-4,IL-5,interferon-γ(INF-γ)levels,and serum dust-mite-specific IgE and IgG4 were determined.Results Five cases were lost in the observation group;0 cases were lost in the control group.The symptom scores and signs scores were significantly lower in the observation group than that in the control group(t=16.307,P<0.01;t=57.284,P<0.01).The IL-2,IFN-γof observation group were significantly higher than those of the control group(t=7.579,P<0.01);(t=17.618,P<0.01),the IL-4,IL-5 of the observation group were significantly lower than those of the control group(t=25.472,P<0.01;t=16.873,P<0.01).After treatment,IgE of the observation group was lower than that of the control group(t=9.708,P<0.01),and IgG4 of the observation group was higher than that of the control group(t=35.690,P<0.01).Conclusion Dust mite allergen combined with low-temperature plasma technology treatment of patients with OSAHS and allergic rhinitis can promote the immune function,reduce IL-4,IL-5 and IgE level,elevate IL-2,IFN-γand IgG4 level,correct the Th1/Th2 immune imbalance,improve immune status and improve the effectiveness of treatment.
