中老年糖尿病治疗过程中低血糖症诱发原因研究及防治策略

摘　　要：目的探讨中老年糖尿病患者在治疗过程中发生低血糖症的原因以及防治策略。方法抽取2018年6月—2019年6月在该院治疗过程中发生低血糖症中老年糖尿病患者78例为研究对象,回顾分析这些患者的临床资料,分析中老年糖尿病治疗过程中低血糖症诱发原因,并提出有效的解决对策。结果78例患者中,22例患者因未及时用餐引起,7例患者因进餐不足引起,9例患者由于静脉滴注胰岛素所致,3例患者由饮酒引起,3例患者合并上呼吸道感染,3例患者合并胃肠炎,31例由于联合用药不规范所导致;所有患者经过静滴葡萄糖、调整饮食习惯、规范用药后低血糖症状均缓解,患者的意识恢复正常状态,治疗过程中无患者死亡;治疗后患者空腹血糖(7.8±0.5)mmol/L、餐后2 h血糖(8.5±1.2)mmol/L均高于治疗前(5.4±0.4)mmol/L、(5.2±0.9)mmol/L,差异有统计学意义(t=4.257、5.278,P<0.05)。结论中老年糖尿病治疗过程中低血糖症原因有患者饮食不当、用药不规范、生活习惯不规律所致,在治疗过程中要根据患者的诱病因素对症下药。Objective To explore the causes of hypoglycemia in middle-aged and elderly patients with diabetes during treatment and the strategies for prevention and treatment.Methods A total of 78 middle-aged and elderly diabetic patients with hypoglycemia during the course of treatment in the hospital from June 2018 to June 2019 were selected as the research objects.The clinical data of these patients were retrospectively analyzed and the causes of hypoglycemia during the treatment of middle-aged and elderly diabetes was analyzed,and effective countermeasures for its solution was proposed.Results Of the 78 patients,22 patients were caused by inadequate meals,7 patients were caused by inadequate meals,9 patients were caused by intravenous drip of insulin,3 patients were caused by drinking,3 patients had upper respiratory tract infection,31 patients had gastroenteritis,and 31 cases were caused by unregulated combination medication;all patients were relieved of symptoms of hypoglycemia after intravenous glucose infusion,dietary adjustment,and standard medication,and the patient's consciousness returned to normal,and no patient died during treatment;the fasting blood glucose(7.8±0.5)mmol/L and the 2 h blood glucose after meal(8.5±1.2)mmol/L after treatment were higher than(5.4±0.4)mmol/L and(5.2±0.9)mmol/L before treatment,the difference was statistically significant(t=4.257,5.278,P<0.05).Conclusion The causes of hypoglycemia in the treatment of middle-aged and elderly patients with diabetes were caused by improper diet,irregular medication,and irregular lifestyle habits.In the course of treatment,the patients should be prescribed according to the inducement factors of the patients.
