摘　　要：目的:分析超声引导下肢神经阻滞复合右美托咪定持续泵注在老年重危患者髋关节置换术中的应用价值。方法:选择2017年12月~2018年12月在本院进行髋关节置换术的老年重危患者120例,随机分成对照组和观察组各60例,对照组选用全麻复合右美托咪定静脉泵注,观察组通过超声引导下肢神经阻滞复合右美托咪定持续泵注,比较不同时段两组患者HR和MAP水平,对比术后两组患者VAS评分。结果:观察组T1、T2时间段HR、MAP水平低于对照组(P<0.05)。观察组术后12h和术后24h的VAS评分低于对照组(P<0.05)。结论:超声引导下肢神经阻滞复合右美托咪定持续泵注在天津市西青医院老年重危患者髋关节置换术中发挥出十分好的麻醉效果,临床价值显著。Objective:To analyze the application value of ultrasound-guided lower limb nerve block combined with continuous pumping of dexmedetomidine in hip arthroplasty in elderly critically ill patients.Methods:A total of 120 elderly critically ill patients who underwent hip arthroplasty in our hospital from December 2017 to December 2018 were randomly divided into a control group and an observation group with 60 cases each.The control group used general anesthesia combined with dexmedetomidine Intravenous pump injection,the observation group was guided by ultrasound-guided lower limb nerve block combined with continuous pump injection of dexmedetomidine.The HR and MAP levels of the two groups of patients were compared at different times,and the VAS scores of the two groups were compared.Results:The levels of HR and MAP in the observation group during T1 and T2 were lower than those in the control group(P<0.05).The VAS scores of the observation group at 12h and 24h after surgery were lower than those of the control group(P<0.05).Conclusion:Ultrasound guided lower extremity nerve block combined with continuous pumping of dexmedetomidine exerts a very good anesthesia effect in the hip replacement of elderly criticallycritical patients in Tianjin Xiqing Hospital,and has significant clinical value.
