腘窝囊肿不同关节镜术式的临床疗效比较

摘　　要：目的探讨腘窝囊肿的关节镜后内侧入路切除术和关节镜后内侧入路囊肿内引流术的临床疗效。方法选取2015年7月至2018年7月泰安市中心医院收治的76例腘窝囊肿患者,分为实验组(40例)和对照组(36例)。实验组采用后内侧入路囊肿切除术,对照组采用囊肿内引流术,观察术后的治疗效果、膝关节功能恢复、囊肿复发及感染情况。结果术后半年和9月时患者膝关节功能评分及手术效果实验组显著优于对照组,差异有统计学意义(P<0.05);术后感染率二者相当,差异无统计学意义(P>0.05);术后1年时复发率实验组明显低于对照组,差异有统计学意义(P<0.05)。结论两种关节镜术式对腘窝囊肿均有较好的临床疗效,后内侧入路腘窝囊肿切除术的临床效果更佳。Objective:To investigate the clinical effects of arthroscopic popliteal cyst cystectomy with the posterior medial approach and popliteal cyst valve resection with internal drainage in the treatment of popliteal cyst.Methods:Seventy-six patients with popliteal cysts,in the Department of Joint and Sports Medicine of Taian City Central Hospital from July 2015 to July 2018 were selected and randomly divided into the experiment group(40 cases)and control group(36 cases).The experiment group adopted the arthroscopic popliteal cyst cystectomy with the posterior medial approach,while control group adopted had popliteal cyst valve resection with internal drainage.The therapeutic effect and the knee joint function and recurrence and infection were observed.Results:The knee joint function score and operation clinical effect in the experimental group were significantly better than those in the control group in postoperative 6 months and 9 months,moreover,the postoperative infection rate in the experimental group and in control group were at the same level,and the difference was not statistically significant(P>0.05).The recurrence rate at 1 year after operation in the experimental group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:The two kinds of arthroscopic treating methods both have good clinical effect on treating popliteal cyst,and the effect of arthroscopic popliteal cyst cystectomy with the posterior medial approach is better.
