摘　　要：目的:探讨成人梭形细胞横纹肌肉瘤(Spindle cell rhabdomyoscarcoma,SCRM)的病理学特征,为临床病理诊断提供了线索。方法:本研究应用免疫组化指标对比分析梭形细胞横纹肌肉瘤与纤维肉瘤、平滑肌肉瘤、恶性纤维组织细胞瘤等恶性间叶组织肉瘤相鉴别,并对比分析各免疫组化指标在其诊断中的敏感性与特异性。回顾性分析某院5年来9例成年人椎体旁梭形细胞癌横纹肌肉瘤的临床组织学特征并比较免疫表型的差异,对比分析各种免疫组化指标的敏感性、特异性与形态学特点之间的关系。结果:肉眼观察肿瘤边界欠清,未见明显包膜;切面灰白色并质韧,肿瘤由条索状排列的梭形细胞组成,胞质深嗜伊红色,部分区域可见散在的横纹肌母细胞;瘤细胞间质内有少量胶原纤维,可见核分裂象。各免疫组化指标的对比分析找出相对特异性与敏感性俱佳的免疫指标,为临床提供相关诊断线索。结论:梭形细胞横纹肌肉瘤是横纹肌肉瘤中一种少见亚型,常发生于婴幼儿,成人发病相对少见。Objective:To investigate the pathological features of spindle cell Rhabdomyosarcoma(SCRM),and to provide the basis for clinical pathological diagnosis.Methods:In this study,immunohistochemical indexes are used to compare spindle cell rhabdomyosarcoma,fibrosarcoma,leiomyosarcoma,malignant fibrohistiocytoma and other malignant mesenchymal tissue sarcomas,and the sensitivity and specificity of each immunohistochemical index in its diagnosis are compared and analyzed.The clinical histological features of rhabdomyosarcoma in 9 cases with parathyroid spindle cell carcinoma in affiliated hospital were retrospectively analyzed and the differences of immunophenotypes were compared.The relationship among the sensitivity,specificity and morphological characteristics of various immunohistochemical indicators was compared and analyzed.Results:The margin of the tumor was not clear and there was no obvious capsule.The cut surface was greyish white and leathery.The tumor was composed of fusiform cells arranged in cords,the cytoplasm was deep eosinophilic in color,and scattered rhabdomyoblasts could be seen in some areas.A few collagenous fibers were found in the stroma of the tumor cells,and and the mitosis could be seen.The relative specificity and sensitivity of the immune indexes were found by comparing and analyzing the immunohistochemical indexes to provide clinical diagnostic clues.Conclusion:Spindle cell rhabdomyosarcoma is a rare subtype of rhabdomyosarcoma,which usually occurs in infants and is relatively rare in adults.
