聚焦超声消融治疗阔韧带子宫肌瘤初步研究

摘　　要：目的探索聚焦超声消融治疗阔韧带子宫肌瘤的可行性、安全性和有效性,为临床治疗方案的选择提供依据。方法分析在镇静、镇痛下进行聚焦超声消融治疗的10例11个阔韧带子宫肌瘤患者,治疗前均经增强MRI确诊。患者年龄36~46岁,平均(37.7±3.1)岁,肌瘤平均最大径约为(6.2±2.3)cm(范围4.2~9.1 cm)。治疗后24 h内、6个月或/和12个月分别行增强MRI复查,测量子宫肌瘤体积、无灌注区体积并计算消融率评估疗效,同时进行安全性评价。结果10例11个阔韧带子宫肌瘤的聚焦超声消融均一次技术成功。肌瘤的平均消融率约(70.8±10.5)%。6个月或/和12个月随访肌瘤缩小分别约(53.5±14.7)%和(71.5±11.6)%。治疗后患者出现骶尾部疼痛3例、下腹部疼痛7例、皮肤灼痛6例,所有患者均未出现严重不良反应。结论聚焦超声消融治疗阔韧带子宫肌瘤是可行、安全和有效的可选择的方法。Objective To investigate the feasibility,safety and efficacy of ultrasound ablation in the treatment of broad ligament leiomyomas,and to provide a basis for the selection of clinical treatment options.Methods Ten patients under conscious sedation with 11 broad ligament leiomyomas in total treated by Haifu JC focused ultrasound tumor therapeutic system were enrolled in retrospective analysis.All patients were diagnosed with enhanced MRI before surgery.The age of the patients was about(37.7±3.1)years(36-46 years),and the average longest diameter of fibroids is about(6.2±2.3)cm(range 4.2-9.1 cm).Enhanced MRI was performed within 24 hours,6 months,and/or 12 months after treatment.The volume of uterine fibroids,the volume of the non-perfusion zone and the ablation rate were calculated,and the safety evaluation and treatment effectiveness evaluation were performed.Results Focused ultrasound ablation of 11 broad ligament fibroids in 10 patients was successful at one time.The ablation rate of fibroids is about(70.8±10.5)%.At 6 or 12 months of follow-up,the fibroid volume decreased by approximately(53.5±14.7)%and(71.5±11.6)%,respectively.After treatment,7 cases of lower abdominal pain,3 cases of palate tail pain,and 6 cases of skin burning pain occurred.No severe adverse reactions occurred in all patients.Conclusions Focused ultrasound ablation is a feasible,safe and effective method for the treatment of broad ligament leiomyomas.
