摘　　要：目的探讨卵巢浆液性交界性肿瘤与卵巢浆液性囊腺瘤的超声特征,分析超声在卵巢浆液性交界性肿瘤与卵巢浆液性囊腺瘤的鉴别诊断价值。方法回顾我院经手术病理证实的13例卵巢浆液性交界性肿瘤及16例卵巢浆液性囊腺瘤临床特征及超声表现。分析年龄、血清CA125、肿块大小、肿块回声、内部分隔、彩色多普勒血流及腹水在两类疾病间的差异性。结果卵巢浆液性交界性肿瘤血清CA125水平(243.73 U/ml)高于卵巢浆液性囊腺瘤血清CA125水平(14.89 U/ml)(t=-3.964,P<0.05)。卵巢浆液性交界性肿瘤93.3%(14/15)为囊实性,6.7%(1/15)为囊性。卵巢浆液性囊腺瘤100%(17/17)为囊性。两组患者肿块回声呈囊实性差异有统计学意义(χ^2=28.207,P<0.05)。超声诊断卵巢浆液性交界性肿瘤的灵敏度、特异度、阳性预测值和阴性预测值依次为92.3%、100.0%、100.0%和94.1%。结论卵巢浆液性交界性肿瘤超声表现多样,大部分超声征象与卵巢浆液性囊腺瘤重叠。当超声发现卵巢囊肿内有单个或多个乳头时,结合患者血清CA125轻度升高应警惕卵巢浆液性交界性肿瘤可能,避免误诊为卵巢良性肿瘤。Objective To explore the ultrasound characteristics of ovarian serous borderline tumor and ovarian serous cystadenoma,and to analyze the value of ultrasound in the differential diagnosis of ovarian serous borderline tumor and ovarian serous cystadenoma.Methods 13 cases of ovarian serous borderline tumor and 16 cases of ovarian serous cystadenoma were reviewed.To analyze the differences of age,serum CA125,tumor size,tumor echo,internal separation,color Doppler flow and ascites between the two diseases.Results The serum CA125 level of ovarian serous borderline tumor(243.73 U/ml)was higher than that of ovarian serous cystadenoma(14.89 U/ml)(t=-3.964,P<0.05).93.3%(14/15)of the serous borderline ovarian tumors were solid and cystic,and 6.7%(1/15)were cystic.Ovarian serous cystadenoma was 100%(17/17)cystic.There was significant difference between the two groups(χ^2=28.207,P<0.05).The sensitivity,specificity,positive predictive value and negative predictive value were 92.3%,100.0%,100.0%and 94.1%respectively.Conclusions The sonographic features of ovarian serous borderline tumors are various,most of which overlap with ovarian serous cystadenoma.When single or multiple papillae are found in ovarian cysts by ultrasonography,combined with the slight increase of serum CA125 in patients,we should pay attention to the possibility of serous borderline tumor of ovary to avoid misdiagnosis as benign tumor of ovary.
