摘 要:目的探讨动脉灌注酸性成纤维细胞生长因子(aFGF)对兔股骨头缺血性坏死的影响。方法用液氮冷冻法制备兔股骨头缺血性坏死模型(全部左侧)。按照体重将造模成功兔随机分为3组:空白对照组、阳性对照组和实验组,每组15只。空白对照组,灌注0.9%NaCl;阳性对照组,灌注尿激酶1×104 U·kg^-1+丹参0.7 mL·kg^-1+罂粟碱1 mg·kg^-1;实验组灌注aFGF 20μg·kg^-1。均连续5 d。灌注后28 d,再次造影观察股骨头坏死区新生血管密度并进行骨密度检测,然后将兔处死,解剖分术侧股骨头并进行组织切片苏木精伊红染色,显微镜下观察空骨陷窝数。结果空白对照组、阳性对照组和实验组骨密度分别为(0.42±0.04),(0.62±0.03)和(0.71±0.04)g·cm-3;这3组的骨细胞空骨陷窝数分别为21.2±2.1,16.8±2.4和12.6±1.8(个/高倍视野)。上述指标:实验组与空白对照组和阳性对照组比较,差异均有统计学意义(均P<0.05)。结论动脉灌注aFGF可改善兔股骨头坏死区域血液供应,促进血管及骨组织再生。Objective To investigate the effect of arterial infusion of acidic fibroblast growth factor(aFGF)on avascular necrosis of femoral head in rabbits.Methods Rabbit models of avascular necrosis of femoral head(all left)were prepared by liquid nitrogen freezing method.Successful rabbits model were randomly divided into three groups according to weight:blank control group,positive control group and experimental group,15 rabbits in each group.The rabbits in blank control group were microcatheterized and perfused with saline;the positive control group was perfused with urokinase 1×104 U·kg^-1+salvia miltiorrhiza 0.7 mL·kg^-1+papaverine1 mg·kg^-1;the experimental group was perfused with aFGF 20μg·kg^-1,for 5 d.Neovascular density of femoral head necrosis was observed by angiography 28 days after perfusion and bone mineral density was measured.Then the rats were sacrificed,and the femoral head was dissected and sectioned for hematoxylin eosin staining.The empty bone lacunae was observed under the microscope.Results Bone mineral density in blank control group,positive control group and experimental group were(0.42±0.04),(0.62±0.03)and(0.71±0.04)g·cm-3,respectively;the number of empty bone lacunae of osteocytes in the three groups were 21.2±2.1,16.8±2.4 and 12.6±1.8(high power field),respectively.Comparison between experimental group and blank control group,positive control group,the difference of the factors were significant(all P<0.05).Conclusion Intra-arterial infusion of aFGF can improve the blood supply of femoral head necrosis and promote the regeneration of blood vessels and bone tissue in rabbits.
