经阴道分娩初产妇产后出血高危因素的调查分析与防范策略

摘　　要：目的探讨经阴道分娩初产妇产后出血的高危因素,制定针对性预防护理策略。方法选择我院2018年3月至2019年9月收治的经阴道分娩初产妇120例,采取回顾分析法收集其一般资料,统计产后出血产妇例数,确定阴道分娩初产妇产后出血的高危因素。结果多因素logistic回归分析显示,流产史、妊娠期高血压、第三产程≥15 min、宫缩乏力、软产道损伤、贫血、低置胎盘、胎盘粘连、巨大儿、催产素使用是经阴道分娩初产妇产后出血的高危因素。结论阴道分娩初产妇伴有产后出血风险,其高危因素有流产史、妊娠期高血压、第三产程≥15 min、宫缩乏力、软产道损伤、贫血、低置胎盘、胎盘粘连、巨大儿、催产素使用,应采取风险预警防控,加强临床护理干预,改善产妇预后。Objective To explore the high-risk factors of postpartum hemorrhage in primiparas undergoing vaginal delivery and develop targeted preventive nursing strategies.Methods We selected 120 cases of primiparas undergoing vaginal delivery admitted to our hospital from March 2018 to September 2019,and general data were collected by retrospective analysis,the number of cases of postpartum hemorrhage was counted,and the high risk factors for postpartum hemorrhage of primiparas undergoing vaginal delivery were determined.Results Multivariate logistic regression analysis showed that the history of miscarriage,hypertensive disorder complicating pregnancy,third stage of labor≥15 min,uterine weakness,soft birth canal injury,anemia,low-lying placenta,placental adhesions,macrosomia,and use of oxytocin were the high risks factors of postpartum hemorrhage in primipara undergoing vaginal delivery.Conclusion The primipara undergoing vaginal delivery was associated with the risk of postpartum hemorrhage.The high risk factors included the history of miscarriage,hypertensive disorder complicating pregnancy,third stage of labor≥15 min,uterine weakness,soft birth canal injury,anemia,low-lying placenta,placental adhesions,macrosomia,and the use of oxytocin,so risk warning and prevention should be adopted,and clinical nursing intervention should be strengthened to improve the maternal prognosis.
