X线引导下恶性肿瘤患者胸壁输液港植入的临床分析

摘　　要：目的探讨X线引导下胸壁输液港植入过程中导丝、导管异位及其他并发症。方法回顾性分析2016年1月—2018年7月在X线引导下行中心静脉输液港植入术的1 141例恶性肿瘤患者,评估穿刺过程中导丝及导管异位、导管通畅率、术后并发症情况。结果各穿刺入路的导丝异位50例,不同穿刺入路的导丝异位率比较差异有统计学意义(P <0.001);导管异位2例,感染1例,气胸1例,导管堵塞3例,术区疼痛2例,血肿2例。结论输液港对肿瘤患者是安全、有效的静脉通路,X线下行中心静脉输液港植入术的安全性较好。Objective To investigate the ectopia rates of catheter or guide wire,and complications of the totally implantable venous access port in patients with malignant tumor under X-ray guidance.Methods From January 2016 to July 2018,1141 patients underwent totally implantable venous access port were retrospectively analyzed.All procedures were guided by X-ray.Ectopia of the guide wire or catheter into an unintended vein,and complications were retrospectively analyzed.Results There were 50 ectopia of the guide wire in different anatomic puncture approach.There were significant differences in the heterotopic rate of guide wire in different puncture approaches(P<0.001).There were 2 cases of ectopic catheter,1 case of infection,1 case of pneumothorax,3 cases of catheter blockage,2 cases of pain and 2 cases of hematoma.Conclusion Totally implantable venous access port is safe and effective for malignant patient.X-ray guided central implantable venous access port is safe.
