三角吻合术联合全腹腔镜下远端胃切除术在胃癌中的应用

摘　　要：目的探讨三角吻合术用于胃癌患者全腹腔镜下远端胃切除术的效果。方法选择本院2016年8月—2019年11月收治住院进行手术治疗胃癌的92例患者作为观察对象,根据手术方法的不同进行分组,将全腹腔镜下远端胃切除术联合三角吻合术治疗的患者纳入试验组(n=46),将腹腔镜辅助远端胃切除术联合小切口常规圆形吻合治疗的患者纳入对照组(n=46)。观察和比较两组患者的治疗效果。结果试验组患者术后吻合口并发症发生率低于对照组,差异有统计学意义(P<0.05);试验组远端切缘距病灶距离短于对照组,视觉模拟疼痛评分低于对照组,消化道重建时间短于对照组,差异均有统计学意义(P<0.05)。结论采用三角吻合术联合全腹腔镜下远端胃切除术可有效提高患者的治疗效果,不但可缩短消化道重建时间,而且能降低术后吻合口并发症发生率,减轻患者术后疼痛,安全性较高。Objective To investigate the effect of trigonostomy combined with total laparoscopic distal gastrectomy for gastric cancer.Methods 92 patients with gastric cancer admitted to our hospital from August 2016 to November 2019 were selected as the observation objects.According to the different surgical methods,the patients were divided into the experimental group(n=46),and the patients with laparoscopic assisted distal gastrectomy combined with small incision conventional circular anastomosis were included in the control group(n=46).The therapeutic effects of the two groups were observed and compared.Results The incidence of postoperative anastomotic complications in the experimental group was lower than that in the control group,the difference was statistically significant(P<0.05);the distance between the distal cutting edge and the lesion in the experimental group was shorter than that in the control group,the visual analogue pain score was lower than that in the control group,and the digestive tract reconstruction time was shorter than that in the control group(P<0.05).Conclusion Trigonostomy combined with total laparoscopic distal gastrectomy can not only shorten the time of digestive tract reconstruction,but also reduce the incidence of postoperative anastomotic complications,reduce postoperative pain and improve the safety of patients.
