床边徒手置入鼻肠管在危重症患者中的应用

摘　　要：目的探讨床边徒手置入鼻肠管与胃镜引导置管法在危重症患者中的有效性及安全性。方法选择2018年2月—2019年9月在本院行营养支持的64例危重症患者为研究对象,按随机数字表法分为两组,各32例,对照组使用胃镜引导置管法,研究组使用床边徒手置入鼻肠管法,对比两组置管成功率、置管费用。结果研究组置管成功率为93.75%(30/32),对照组置管成功率为100%(32/32),差异无统计学意义(P> 0.05);研究组置管费用为(493.36±6.98)元,低于对照组的(1 340.78±9.48)元,差异有统计学意义(P <0.05)。结论床边徒手置入鼻肠管与胃镜引导置管法均是危重症患者营养支持的有效置管方式,但前者可减少置管费用,减轻患者经济负担,值得临床推广。Objective To explore the efficacy and safety of bedside nasointestinal tube placement by bare hand and gastroscope guided tube placement in critically ill patients.Methods 64 critically ill patients who received nutritional support in our hospital from February 2018 to September 2019 were selected as the study objects,and divided into two groups according to the random number table method,32 cases in each group.The control group used gastroscope guided catheterization,and the study group used bedside manual catheterization.The success rate and cost of the two groups were compared.Results The success rate of catheterization was 93.75%(30/32)in the study group and 100%(32/32)in the control group,with no significant difference(P>0.05).The cost of catheterization in the study group was(493.36±6.98)yuan,lower than that in the control group(1340.78±9.48)yuan,the difference was statistically significant(P<0.05).Conclusion The nasogastric tube inserted by bare hand at the bedside and the gastroscope guided tube placement are both effective ways for nutrition support of critical patients,but the former can reduce the cost of tube placement and reduce the economic burden of patients,which is worthy of clinical promotion.
