高频超声与CT对甲状腺乳头状癌的诊断价值

摘　　要：目的对比高频超声与CT对甲状腺乳头状癌的临床诊断价值。方法采集本院2016年7月—2019年12月经临床病例确诊甲状腺乳头状癌的79例患者,分析患者术前高频超声和CT检查的影像学资料,比较两种检查方法的诊断准确率。结果手术病理确诊颈部淋巴结转移37例,高频超声检出颈部淋巴结转移13例,检出率35.14%,CT检查检出颈部淋巴结转移24例,检出率64.86%,高频超声颈部淋巴结检出率较CT低,差异有统计学意义(P <0.05);高频超声对甲状腺乳头状癌诊断准确率为87.80%,与CT诊断准确率89.02%比较,差异无统计学意义(P> 0.05),高频超声联合CT诊断准确率为97.56%,显著高于单项检查,差异有统计学意义(P <0.05)。结论高频超声联合CT检查能提高甲状腺乳头状癌的诊断准确率,是适合临床诊断的重要手段。Objective To compare the clinical value of high frequency ultrasound and CT in the diagnosis of thyroid papillary carcinoma.Methods From July 2016 to December 2019,79 patients with papillary thyroid cancer were collected.The imaging data of preoperative high-frequency ultrasound and CT were analyzed,and the diagnostic accuracy of the two methods was compared.Results There were 37 cases with cervical lymph node metastasis confirmed by operation and pathology,13 cases with cervical lymph node metastasis detected by high frequency ultrasound,the detection rate was 35.14%,24 cases with cervical lymph node metastasis detected by CT,the detection rate was 64.86%,and the detection rate of cervical lymph node by high frequency ultrasound was lower than that by CT,the difference was statistically significant(P<0.05).In the diagnosis of papillary thyroid carcinoma,there was no significant difference between the accuracy of high frequency ultrasound(87.80%)and CT(89.02%)(P>0.05).The accuracy of high frequency ultrasound combined with CT was 97.56%,which was significantly higher that of single examination,the difference was statistically significant(P<0.05).Conclusion High frequency ultrasound combined with CT can improve the diagnostic accuracy of papillary thyroid cancer,and is an important method for clinical diagnosis.
