摘 要:目的:运用多普勒超声技术检测计算正常人颈总动脉、桡动脉、足背动脉的牛顿射血力,探讨将超声技术引入脉象力学分析的可行性。方法:纳入45例健康者,将多普勒超声探头分别置于受试者双侧颈总动脉,双侧桡动脉及双侧足背动脉,测量以上动脉的收缩期血管内径、血流加速度及加速期流速积分,根据牛顿第二定律F=ma计算出牛顿射血力。结果:45例健康者双侧颈总动脉的平均血管内径为(0.64±0.09)cm,血管截面积为(1.29±0.37)cm^2,血流加速度为(851.27±512.80)cm/s^2,加速期流速积分为(4.18±1.33)cm,牛顿射血力为(1189.73±497.59)dynes。双侧桡动脉的平均血管内径为(0.25±0.10)cm,血管截面积为(0.24±0.24)cm^2,血流加速度为(551.35±174.00)cm/s^2,加速期流速积分为(2.35±0.91)cm,牛顿射血力为(98.50±128.57)dynes。双侧足背动脉的平均血管内径为:(0.24±0.05)cm,血管截面积为:(0.20±0.09)cm^2,血流加速度为(586.07±427.85)cm/s^2,加速期流速积分为(2.50±0.96)cm,牛顿射血力为(91.92±115.26)dynes。正常人双侧人迎脉(颈总动脉)与双侧寸口脉(桡动脉)、双侧趺阳脉(足背动脉)各参数之间比较,差异有统计学意义(P<0.05)。双侧颈总动脉处的血管内径、截面积、加速度流速积分、血流加速度及牛顿射血力均明显高于双侧桡动脉及双侧足背动脉。双侧寸口脉(桡动脉)与双侧趺阳脉(足背动脉)各参数之间比较差异无统计学意义(P>0.05)。结论:通过检测颈总动脉、桡动脉、足背动脉的牛顿射血力可对人迎、寸口、趺阳脉的力学特征分析提供客观指标,血管内径、血流加速度及加速期流速积分对牛顿射血力的计算值影响较大。Objective:To discuss the feasibility of introducing ultrasonic technology into pulse mechanics analysis through detecting and calculating Newton′s ejection force of common carotid artery,radial artery and dorsal pedis artery in normal people.Methods:After recruiting forty-five normal people,Doppler ultrasound probe was placed on both sides of common carotid artery,radial artery and dorsal pedis artery in normal people,to detect systolic vascular diameter,blood flow acceleration and velocity integral in acceleration period of these arteries,according to newton′s second law F=ma,to calculate Newton′s ejection force.Results:Mean vessel diameter of bilateral common carotid arteries in 45 normal people was(0.64±0.09)cm,cross-sectional area of blood vessel was(1.29±0.37)cm^2,blood flow acceleration(851.27±512.80)cm/s^2,velocity integral in acceleration period(4.18±1.33)cm,Newton′s ejection force(1189.73±497.59)dynes.Mean vessel diameter of bilateral radial arteries was(025±0.10)cm,cross-sectional area of blood vessel was(0.24±0.24)cm^2,blood flow acceleration(551.35±174.00)cm/s^2,velocity integral in acceleration period(2.35±0.91)cm,Newton′s ejection force(98.50±128.57)dynes.Mean vessel diameter of bilateral dorsal pedis arteries in 45 normal people was(0.24±0.05)cm,cross-sectional area of blood vessel was(0.20±0.09)cm^2,blood flow acceleration(586.07±427.85)cm/s^2,velocity integral in acceleration period(2.50±0.96)cm,Newton′s ejection force(91.92±115.26)dynes.The difference had statistical meaning in different parameters of bilateral Renying pulse(common carotid artery),bilateral Cunkou pulse(radial artery)and bilateral Fuyang pulse(dorsal pedis artery)of normal people.Vessel diameter and cross-sectional area,velocity integral in acceleration period and Newton′s ejection force of bilateral common carotid artery were higher than these of bilateral radial artery and bilateral dorsal pedis arteries notably.The difference had no statistical meaning in different parameters of bilateral Cunkou p
