调经消异汤对子宫内膜异位症术后患者的影响

摘　　要：目的:观察调经消异汤联合腹腔镜术治疗子宫内膜异位症(endometriosis,EMT)的临床疗效及对术后患者血清糖类抗原125和199(CA125、CA199)及肿瘤坏死因子α(tumor necrosis factorα,TNF-α)的影响。方法:将110例患者随机分为两组,每组55例。两组均行腹腔镜保守手术治疗,观察组同时服用调经消异汤治疗4个月经周期,观察两组治疗前后中医证候积分、CA125、CA199、TNF-α、黄体生成素(luteinizing hormone,LH)、卵泡刺激素(follicle stimulating hormone,FSH)、雌二醇(estradiol,E2)、疼痛评分和临床疗效。结果:观察组总有效率为90.91%(50/55),对照组为78.18%(43/55),两组比较差异有统计学意义(P<0.05)。中医证候积分、疼痛评分两组治疗后均较治疗前有所下降(P<0.05),且观察组下降程度大于对照组(P<0.05)。CA125、CA199、TNF-α、LH、FSH及E2两组治疗后均较治疗前有所下降(P<0.05),且观察组下降程度大于对照组(P<0.05)。结论:调经消异汤联合腹腔镜术治疗EMT患者,临床疗效显著,能显著降低EMT患者中医证候积分,有效降低CA125、CA199及TNF-α含量。Objective:To observe clinical effects of period-regulating endometriosis-eliminating decoction combined with laparoscopy in treating endometriosis(EMT)and its influence on serum CA125,CA199 and TNF-α.Methods:All 110 patients were randomized into two groups,55 cases each group.Both groups were treated by laparoscopic conservative surgery,the observation group took period-regulating endometriosis-eliminating decoction for four menstrual cycles simultaneously,to observe TCM syndrome integrals,serum CA125,CA199,TNF-α,LH,FSH,E2,VAS and clinical effects before and after treating between both groups.Results:Total effective rate of the observation group was 90.91%(50/55),higher than 78.18%(43/55)of the control group,and the difference was statistically significant(P<0.05).TCM syndrome integrals and VAS in both groups after treating reduced than before treating(P<0.05),and the decrease of the observation group was greater than that of the control group(P<0.05).CA125,CA199,TNF-α,LH,FSH and E2 in both groups after treating decreased compared with these before treating(P<0.05),and the decrease of the observation group was greater than that of the control group(P<0.05).Conclusion:Period-regulating endometriosis-eliminating decoction combined with laparoscopy could obtain notable clinical effects in treating EMT patients,notably lower TCM syndrome integrals of EMT patients,effectively reduce the contents of CA125,CA199 and TNF-α.
