清肺十八味丸联合桉柠蒎对老年COPD合并PTB患者肺功能及炎性因子水平的影响

摘　　要：目的:探讨清肺十八味丸联合桉柠蒎对老年慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)合并肺结核(pulmonary tuberculosis,PTB)患者肺功能及炎性因子水平的影响。方法:将老年COPD合并PTB患者190例根据硬币投掷法随机分为对照组和研究组各95例,对照组患者给予桉柠蒎治疗,研究组患者在对照组治疗的基础上口服清肺十八味丸治疗,比较两组患者治疗效果、治疗前后肺功能及血清炎症因子水平。结果:研究组总有效率为92.6%(88/95),高于对照组的76.8%(73/95)(P<0.05);治疗后两组患者症状积分较治疗前降低,且研究组低于对照组(P<0.05);研究组患者肺功能恢复时间及肝肾损害率低于对照组(P<0.05),痰菌转阴率高于对照组(P<0.05);治疗后两组患者肺功能指标及各炎性因子水平较治疗前改善,且研究组改善优于对照组(P<0.05)。结论:清肺十八味丸联合桉柠蒎可有效改善老年COPD合并PTB患者临床症状及肺功能,能抑制机体炎性反应,促进痰菌转阴,且肝肾损害小于桉柠蒎。Objective:To explore the influence of Qingfei Shibawei pills joined with ELP enteric soft capsules on pulmonary function and the levels of inflammatory factors in the elderly with COPD and PTB.Methods:All 190 patients were randomized into the control group and the study group according to coin throwing method,the control group were treated by ELP enteric soft capsules,and the study group took Qingfei Shibawei pills orally based on ELP enteric soft capsules,to compare therapeutic effects,pulmonary function and the levels of inflammatory factors before and after treating between both groups.Results:Total effective rate of the study group was 92.6%(88/95),higher than 76.8%(73/95)of the control group(P<0.05);after treating,symptom scores of both groups reduced than before treating,and the study group was lower than the control group(P<0.05);the study group was lower than the control group in the recovery time of pulmonary function and the rate of liver and renal damage(P<0.05);higher than the control group in sputum bacteria negative conversion rate(P<0.05);after treating,the indexes of pulmonary function and the levels of inflammatory factors in both groups improved compared with these before treating,and the study group was superior to the control group(P<0.05).Conclusion:Qingfei Shibawei pills joined with ELP enteric soft capsules could effectively improve clinical symptoms and pulmonary function of the elderly with COPD and PTB,it could inhibit inflammatory response,promote the negative conversion of sputum bacteria,and its damage to liver and kidney smaller than the effects of ELP enteric soft capsules.
