冠状动脉粥样硬化性心脏病中医证型与血脂、颈动脉粥样硬化相关性研究

摘　　要：目的:探讨冠状动脉粥样硬化性心脏病(coronary heart disease,CHD)患者各中医证型与血脂水平、颈动脉粥样硬化的相关性。方法:对260例已确诊为CHD患者的相关资料进行回顾性分析,记录一般情况、三酰甘油(three acyl glycerin,TG)、血清总胆固醇(total cholesterol,TC)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)、颈动脉内膜中层厚度(intima-media thickness,IMT)及斑块积分,并比较各中医证型与上述指标的关系。结果:CHD患者中医证型以痰浊痹阻证最多,证型有从痰浊痹阻证、心血瘀阻证、气虚血瘀证至气阴两虚证发展趋势;痰浊痹阻证患者的血脂水平、颈动脉IMT及斑块积分明显高于其他证型。结论:将中医证型与血脂、颈动脉IMT结合起来,可更好地指导冠状动脉粥样硬化性心脏病的早期诊治,同时为中医证型的客观化研究提供参考依据。Objective:To probe into the correlation between TCM syndrome types,blood lipid level and carotid atherosclerosis in patients with coronary heart disease(CHD).Methods:The data of 260 patients with CHD were retrospectively analyzed.The general condition,triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C) and high density lipoprotein cholesterol(HDL-C),intima-media thickness(IMT) and plaque score were recorded.The relationship between TCM syndrome types and the above indexes was compared respectively.Results:The CHD patients mainly had phlegm turbid obstruction syndrome.The TCM syndrome types had tendency of changing from phlegm turbid obstruction syndrome,heart blood stasis syndrome,qi deficiency and blood stasis syndrome to qi-yin deficiency syndrome.The blood lipid level,carotid IMT and plaque score of patients with phlegm turbid obstruction syndrome were significantly higher than those of other syndromes.Conclusion:The combination of TCM syndrome types with blood lipid and carotid IMT can better guide the early diagnosis and treatment of CHD,and provide reference for the objective study of TCM syndrome types.
