基于医院等级评审标准数据的综合医院感染管理影响因素分析

摘　　要：目的通过对34家综合医院基本情况及医院感染条款评审数据进行现状描述和综合分析,讨论其合格率的影响因素。方法依据《三级综合医院评审标准(2011年版)》对34家综合医院进行评价,评价结果数据结合"医院基本情况统计表"数据进行汇总分析。结果本次调查的34家医院的医院感染管理相关条款(68项)总体合格率为82.31%,其中医院感染持续改进条款(18项)合格率为90.03%,医院手卫生管理条款(2项)合格率为76.47%,消毒供应中心管理相关条款(7项)合格率为92.67%;医护比(OR=26.30,P=0.019)、医师日均担负住院床日数(OR=20.83,P=0.028)、床位周转次数(OR=47.84,P=0.032)为医院感染管理相关条款合格率的独立影响因素。结论合理统筹、科学化护理人员管理模式,通过调动护理人员工作积极性释放工作效能,增加有效医护比,优化床位资源利用以及改善医生工作负荷量是提高医院感染管理现状的有效手段。Objective To discuss the influencing factors of nosocomial infection(NI)pass rate by describing and analyzing the basic information of 34 comprehensive hospitals and the accreditation data of NI terms.Methods 34 gene ral hospitals were evaluated in accordance to the Tertiary General Hospital Accreditation Criteria(2011 Edition),and accreditation results were examined combined with data from"Statistical Table of Hospital Basic Information".Results The overall pass rate of 34 hospitals surveyed displays an overall pass rate of 82.31%in NI terms(68 articles),with 90.03%,76.47%and 92.67%in NI continual improvement terms(18 articles),hospital hand hygiene management(2 articles)and central sterile supply department management terms(7 articles)respectively.The ratio of doctor over nurse(OR=26.30,P=0.019),average daily inpatient per doctor(OR=20.83,P=0.028)and bed turnover(OR=47.84,P=0.032)were identified as independent influencing factors of NI terms pass rate.Conclusion Effective ways to enhance NI management point are innovating nursing personnel management mode,improving work performance by promoting nurse morale,increasing ratio of doctor over nurse,optimizing the utilization of bed resources and releasing physician workload.
