血清Clara细胞分泌蛋白的表达水平对新生儿ARDS诊断及疗效评估的意义

摘　　要：目的探讨血清Clara细胞分泌蛋白(CCSP)的表达水平对新生儿急性呼吸窘迫综合征(ARDS)诊断及疗效评估的意义。方法选择我院2013年6月至2016年12月新生儿重症监护室收治的51例新生儿ARDS作为ARDS组,根据影像学表现分为Ⅰ级(23例)、Ⅱ级(17例)、Ⅲ级(11例)。另选择正常新生儿39例作为对照组。比较对照组新生儿和ARDS组各分级患儿出生后2 h、3 d、7 d血清CCSP的表达水平。结果对照组出生后2 h的血清CCSP水平高于于ARDS组,且胸片情况愈严重,血清CCSP表达水平愈低,差异具有统计学意义(P<0.001)。ARDS组和对照组出生后3、7 d的CCSP表达水平比较,差异无统计学意义(P>0.05)。结论新生儿ARDS早期的血清CCSP表达水平降低,且患儿胸片结果越严重,CCSP的表达水平越低,对ARDS的早期诊断有一定的参考意义。Objective To investigate the significance of the expression level of serum Clara cell secretory protein(CCSP)in diagnosis and therapeutic evaluation of neonates with acute respiratory distress syndrome(ARDS).Methods Fifty-one neonates with ARDS admitted in neonatal intensive care unit of our hospital from June 2013 to December 2016 were selected as ARDS group and divided into gradeⅠ(23 cases),gradeⅡ(17 cases)and gradeⅢ(11 cases)according to imaging findings.Another 39 normal neonates were selected as control group.The levels of serum CCSP were compared between neonates in the control group and each grade neonates in the ARDS group at 2 h,3 d and 7 d after birth.Results The level of serum CCSP in the control group was higher than that in the ARDS group at 2 h after birth,the more serious the chest X-ray was,the lower the serum CCSP expression level was,and the differences were statistically significant(P<0.001).There was no significant difference in serum CCSP expression level between the ARDS group and the control group at 3 and 7 days after birth(P>0.05).Conclusion The expression level of serum CCSP in the early stage of neonates with ARDS decreases,and the more serious the chest X-ray results is,the lower the expression level of CCSP is,which has certain reference significance for the early diagnosis of ARDS.
